Primärvårdsläkares upplevelser av sin mentala hälsa på vårdcentraler med olika organisationsstrukturer.: En kvantitativ tvärsnittsstudie

Abstract [en]

The increased ill-health of doctors in primary care is a major problem today with the highest possible reduction in the ability to influence their workplace. Although the individual physician can reduce his or her stress after individual actions, it is not sustainable in the long run. There is a risk that one must have a symptom among employees rather than identifying the underlying problem in the organizational structure. The purpose is to examine perceived health, job demands, control and support, job satisfaction and quality of care among doctors at health centers, and compare whether there is any difference in health centers that triage patients via direct booking to the appropriate institution according to the patient's needs in this study: physiotherapist, compared to health centers who always first book a doctor's time for the patient or refer to a physiotherapist. The method were quantitative, cross-sectional. The questionnaires were: - General Health Questinonnarire, which measures self-assessed mental health, - Demand Control Support Questionnarie, measures perceived control, demands, support, and the questionnaire - Satisfaction Work Questionnarie, which measures job satisfaction and quality of care. The result indicates no significant difference between the two types of health centers on the doctors' perceived health.

Abstract [sv]

Den ökade ohälsan hos läkare i primärvården är ett stort problem idag med upplevd minskad möjlighet att kunna påverka sin arbetssituation. Även om den enskilde läkaren kan minska sin stress efter individuella åtgärder är det inte hållbart på sikt. Det finns risk att man åtgärdar symptom hos medarbetare istället för att identifiera det underliggande problemet i organisationsstrukturen. Syfte var att undersöka läkarnas upplevda hälsa, krav, kontroll och stöd, arbetstillfredsställelse och vårdkvalitet på vårdcentraler, och jämföra om det finns någon skillnad hos läkare på vårdcentraler som triagerar patienter via direktbokning till lämplig instans efter patientens behov i denna studie, fysioterapeut, jämfört med läkare på vårdcentraler som alltid först bokar upp en läkartid för patienten eller hänvisar till fysioterapeut. Som metod användes kvantitativ, tvärsnittdesign.  Först gjordes urval av enkätfrågor som bestod av frågor gällande läkarnas upplevda mental hälsa för att svara på frågeställningarna. Frågeformulären var: - General Health Questinonarire (GHQ), som mäter självskattad psykisk hälsa, - Demand Control Support Questionnarie (DSCQ), mäter upplevd kontroll, krav och stöd, och frågeformuläret - Satisfaction Work Questionnarie (SWQ) som mäter arbetstillfredsställelse och vårdkvalitet. Resultatet visar ingen signifikant skillnad mellan de två typerna av vårdcentraler på läkarnas upplevda mentala hälsa.
