"It´s better to die than to lose one´s autonomy": en kunskapsöversikt över äldres autonomi i äldreomsorgen

Abstract [sv]

Syftet med denna selektiva kunskapsöversikt var att belysa de faktorer som kan bidra till att äldre människor känner autonomi och möjlighet att själva påverka sina liv trots att de är i beroendeställning till andra för att klara sin allmänna dagliga levnad. Utifrån den valda forskningslitteraturen fokuserade vi oss på tre teman; formella och informella omsorgsgivares betydelse för autonomi, valmöjlighet/delaktighet samt livskvalitet. Kunskapsöversikten tolkades sedan hermenutiskt. Resultatet påvisade att både informella och  formella omsorgsgivare har en stor betydelse för äldres upplevelse av autonomi, där integritet och självbestämmande var ledorden. Äldre människor upplever inte delaktighet i omsorgssituationer eller handläggningsprocessen. Resultatet påvisade att autonomin påverkas av fysisk hälsa samt upplevelse av att bli bekräftad och ha kontroll över sin egen livssituation, vilka kan ha en inverkan på den upplevda livskvaliteten. Avseende delaktighet och livskvalitet var hemmet en bidragande faktor till autonomi. Resultatet analyserades med hjälp av maktteori samt ålderism.

Abstract [en]

The aim of this literature overview was to map out the factors that can help older people feel autonomy even though they are dependent on others. Based on the selected research literature, we focused on three themes; the informal and formal caregivers importance, choice/participation and quality of life. The results have shown that booth informal and formal caregivers have a significant impact on older people´s experience of autonomy. Elderly people´s autonomy was also affected by physical health and the experience to be acknowledged and have control over their own lives. Both participation, quality of life and being able to remain in their home, was contributing factors to autonomy. Elderly people felt that they were not involved in decision making regarding how help should be organized. The results have been analyzed with the help of power theory and ageism.
