    Abstract [en]

    BACKGROUND: Health-related quality of life (HRQoL) peaks around 1 year after Roux-en-Y gastric bypass (RYGB) surgery, and thereafter, in many patients, slowly deteriorates.

    OBJECTIVES: The aim of the present study was to test early effects (study endpoint 2 years) of a dissonance-based group intervention on HRQoL (primary outcome) and wellbeing among women who underwent RYGB: a 1-year follow-up of the WELL-GBP trial.

    SETTING: Women were recruited from 5 different hospitals in Sweden pre-RYGB surgery. Participants were randomized to intervention or a control group (regular care).

    METHODS: The intervention consisted of 4 group sessions, 2 to 3 months post-surgery, comprising the following 4 different topics: (1) physical activity, (2) eating behavior, (3) social relationships, and (4) intimate relationships. Participants answered questionnaires about HRQoL (SF-36, Short-Form Health Survey), social adjustment, body esteem, eating behavior, and wore an accelerometer for 7 days at pre- and 1 year post-RYGB.

    RESULTS: Two hundred fifty-nine women were recruited and 203 (78%) completed 1-year follow-up measurements. Mean body mass index pre-surgery was 40.8 (standard deviation = 4.5), mean age 44.7 (standard deviation = 10.3) years, and 61 of 120 women in the intervention group received the intervention according to protocol (≥3 group sessions). We observed no difference between the intervention and the control group at 1-year post-RYGB surgery. All scales improved in both groups from pre- to 1 year post-surgery.

    CONCLUSIONS: We did not observe any 1-year early effects on HRQoL from a dissonance-based group intervention among female RYGB patients. Future studies may investigate long-term effects of the intervention.

    A preschool-based dietary survey, using 7-d records, was carried out in a suburb of Stockholm. The aim was to assess the intake of food and the quality of the diet of preschool children aged 3-5 y at preschool and at home, and to compare the dietary intake with the Swedish dietary recommendations for preschool children. The respective mean intakes of protein, fat, carbohydrates and sucrose, expressed as a percentage of total energy intake were 14, 38, 50 and 9 at preschool, and at home 14, 36, 52 and 12 on weekdays, 14, 34, 55 and 16 on weekend days. The mean intakes of seven vitamins and minerals were low only for selenium as compared with the recommended level. No differences were found in nutrient density between diet at preschool and diet at home, with the exception of dietary fibre (higher at preschool). On weekdays there was a significantly higher nutrient density for calcium, zinc, selenium, vitamin A, riboflavin, vitamin C and dietary fibre compared with weekend days. Conclusion: The average intakes of energy and nutrients per meal at preschool compared with the recommended levels for children aged 4-6 y were low for all meals (breakfast, lunch and afternoon snack). This, however, was compensated for by home meals.

    In this research study, the role of social support from the supervisor in a return to work context was examined and analysed. Five individuals on sick leave due to burnout were asked to participate in the study. Informed consent was obtained from each individual and they were then subjected to a standardised interview, which was evaluated using qualitative content analysis. The result shows that individuals with burnout have expectations of social support from the supervisor, in particular with regard to taking responsibility and established a sense safety. The supervisor is expected to structure the rehabilitation and return to work, but also display emotional commitment such as trust, sensitivity and empathy. The communication between the supervisor and the individual during sick leave and the experience of social support from the supervisor may indeed be crucial to mediate a successful recovery. This study emphasized the importance of social support from the supervisor during sick leave from burnout to optimize return to work.

    Tidigare forskning har visat att vi i dagens samhälle är allt mer fysisk inaktiva, samtidigt som kunskapen om betydelsen av fysisk aktivitet är stor. Hälsofrämjande arbete är ett sätt att främja befolkningens hälsa och det finns många faktorer som påverkar hur aktiva vi är. Studiens syfte var att ta reda på hur deltagare i ett hälsoprojekt upplevde att deras fysiska aktivitetsgrad varierade under projektet och vad de upplevde påverkade förändringen. Studien har även undersökt hur de upplevde att motivationen förändrades under projektet. Semi-strukturerad intervju har använts som undersökningsmetod. Urvalet var fem individer som genomfört ett hälsoprojekt. Resultatet visade att social gemenskap har stor betydelse för att utföra fysisk aktivitet. Gemenskapen medförde även att ett förtroende utvecklades som innebar att inte svika de andra i projektet vilket motiverade till fysisk aktivitet. Undersökningen visade även att projektet i sig var positivt för att öka aktivtetsgraden men att den till viss del sjönk i slutet av projekttiden. Andra påverkansfaktorer som nämns är prioriteringen av tid samt avbrott i aktiviteten såsom semester och jul. Vädret påverkade också valet av att vara aktiv eller inte.

    Abstract [en]

    Pain is a complex and challenging problem for patients with low back pain and for the psychologists and physiotherapist. One of the most common musculoskeletal pain problems is low back pain. Literature data allow us to assume that possible cause of decreased multifidus size include reflex inhibition and disuse atrophy. Increased fatigability (decreased endurance) of the multifidus muscle has been demonstrated in low back pain (LBP) patients in several research studies. Taking up physical activity belongs to one of the fundamentals of the therapy Formal exercise programmes used in conjunction with other methods of treatment and psychological therapy are one the main forms treatments in low back pain. Lifestyle, physical activity in particular is one on the factors influencing the health of youth and adults. Passiveness and apathy do not facilitate the persistence in realizing the aims of treatment.

    Abstract [en]

    University teachers' conditions at work have changed and work-related stress increases among academics atuniversities. The aim of this study was to investigate whether university teachers' experience of work-related stress and recovery differed depending on their extent of telework. The study wasan empirical quantitative cross-sectional study where 95 university teachers’ from two universities in Sweden answered a web survey. About half of the university teachers’ telework several times a week and the results show that university teachers' experience of recovery in relation to telework differs, but not their work-related stress. Gender was found to have a small impact on the recovery to telework, but not onwork-related stress. The conclusion is that university teachers’ who telework frequently experience worse recovery than those who rarely telework. The difference in recovery may be because telework makes it more difficult for the university teacher to finish the work compared to they who not telework.

    Skolerfarenheter och självskattad hälsa bland elever i särskolan2018Report (Refereed)
    Abstract [sv]

    Särskolan är en skolform för elever med intellektuella funktionsnedsättningar. Forskning visar att elever i särskolan, trots olika insatser från olika aktörer, inte erbjuds full delaktighet i samhället. En god förståelse för dessa elevers skolgång är därför viktig för att arbeta för de ska få en så tillfredställande livssituation som möjligt. Syftet med projektet var att få ökad kunskap om vilka samband som finns mellan elevernas socioekonomiska bakgrund, sociodemografiska förhållanden, upplevelse av skolsituation samt deras egna skattningar av hälsa. Särskoleelevernas svar jämförs även med svar från elever i övrig skola. Data bygger på enkätundersökningar som genomförts bland 1320 särskoleelever i högstadium och gymnasium i olika kommuner åren 2003-2015. En jämförelse görs även mellan elever i särskolan och elever i övriga skolan i Stockholm för åren 2013-2015.

    Tre frågor har formulerats: Hur upplever elever i grundsärskolan och gymnasiesärskolan sin skolsituation och hälsa? Vilka samband finns mellan särskoleelevernas socio-ekonomiska bakgrund, sociodemografiska förhållanden, upplevelse av skolsituation samt deras självskattade hälsa och egenrapporterade hälsobesvär. Finns det några skillnader mellan elever i särskolan och övrig skola avseende skolsituation och hälsa? Data visar att:

    • Ungefär hälften av eleverna i särskolan skattar sin hälsa som mycket bra, ca 10 % anser att den är dålig.
    • Majoriteten särskoleelever upplever skolklimatet som positivt med goda möjligheter till inlärning.
    • Mer än hälften av eleverna i särskolan har förtroende för lärare
    • För självskattad hälsa framträder inga signifikanta samband med utländsk bakgrund. Bland särskoleelever på högstadiet är psykiska besvär mindre vanliga bland invandrarungdomar. Elever i särskolan med utländsk bakgrund är överrepresenterade jämfört med motsvarande andel i grundskolan och gymnasieskolan.
    • Resultaten visar att kön spelar roll. Ser vi till olika uppskattade hälsobesvär gäller genomgående att dessa är vanligare bland flickor. Detta gäller både särskola och övrig skola.
    • Förtroende för föräldrar och skoltrivsel kan kopplas samman med självskattad hälsa både bland elever i särskolan och bland elever i övriga skolan. Ungdomar som har förtroende för föräldrar skattar sin hälsa högre. För skoltrivsel följer sambandet i huvudsak ett mönster där bättre skoltrivsel kopplas till bättre självskattad hälsa.
    • Bland elever på högstadiet i övriga skolan finns en koppling till familjesituation och socioekonomisk bakgrund. Ungdomar som bor hos båda föräldrar eller har växelvis boende redovisar lägre förekomst av psykiska besvär än ungdomar i andra boenden.
    • Bland eleverna i gymnasiesärskolan framträder ett klart samband med förtroende för lärare. De som inte angett att de har förtroende för lärare redovisar högre förekomst av besvär.

    Det finns några skillnader mellan elever i särskolan och övrig skola avseende skolsituation och hälsa, men de grundläggande dragen är desamma. Sammantaget tyder resultaten på att det är samma faktorer som påverkar hälsa bland elever i särskolan och i övriga skolan. Det centrala är om eleverna har förtroende för sina föräldrar och hur de trivs i skolan.

    Länge har kvinnan ansetts vara svagare både psykiskt och fysiskt därmed har mannen varit överordnad kvinnan under historiens gång. Idag lever vi i ett land som är känt för sitt jämställda samhälle men om vi blickar tillbaka i historien så kan vi se hur kvinnoidealen har förändrats genom tiderna. Idag lever vi ett multikulturellt Sverige där vi glömmer att vi delar vår kultur med omgivningen. Många tjejer lever idag i ett svenskt samhälle med en annan kultur, men frågan är hur det är att leva med en dubbelkultur.

    Denna studie undersöker om en uppväxt med dubbla kulturer bidrar till extra press på unga kvinnor, på grund av det kvinnliga idealet inom enskilda kulturer. Resultatet visade att tjejerna med en dubbelkultur kände sig pressade eftersom de försökte tillfredsställa sina föräldrars önskningar om hur en kvinna bör vara, samtidigt som de unga kvinnorna försökte platsa in och bli accepterade i samhället.

    A validation study was performed in order to compare the observed versus the self-reported food consumption in anorectics (n = 9), anorectic bulimics (n = 6) and bulimics (n = 10). The observed meals (breakfast, lunch and/or dinner) were served in a hospital setting. Foods on plates were unobtrusively weighed before and after serving to each subject, and the foods consumed were compared with data from a 24-hour recall the subsequent day. Data analyses via a nutrient database comprised amounts of food items from seven food groups as well as energy, macro nutrients, vitamin C, calcium and iron. Analyses of recalled intakes showed consistently high correlations with observed intakes, suggesting that the 24-hour recall gives valid results, and could thus be suitable for this category of patients. This notion contrasts with previous statements, although no validation studies have been undertaken in order to support this view

    Psychophysiological reactions, stress and recuperation among telecommuting academics

    Telecommuting is a working arrangement that is highly adopted among academic staff. This may be a cause or a consequence of expanding demands on productivity, efficiency and availability of academic staff. A previous study on white-collar workers at a government authority showed higher cardiovascular arousal during work at the office, but less recovery after working from home. To our knowledge, these findings have not been confirmed or refuted in other populations. As part of an ongoing study, we aimed to determine whether there is a difference in perceived stress and psychophysiological reactions during work at the central workplace (CW) compared to work performed away from the central workplace (OCW) among academic staff. In addition, recuperation after work performed at the two locations were compared. For each subject, saliva cortisol was collected regularly throughout each of two working days in the same work week (i.e., one full day CW and one full day OCW). Before and after each working day, the subjects rated their stress and fatigue on 100 mm Visual Analogue Scales (VAS) where higher values imply more stress and fatigue, and the difference in ratings within a day (i.e., after-before) was calculated. In the morning after each of the working days, the subjects rated how recuperated they felt on VAS where higher values imply more recuperation. Cortisol values were analyzed using repeated measures analysis of variance with Day (2 days) and Time of day (6 time points) as within-subjects effects. Self-ratings were compared using paired-samples T test. In all tests, the level of significance was p=0.05. Preliminary results show no significant differences between days in cortisol curves (F=0.62; p=0.685), stress (Dstress=2 for CW and -1 for OCW; p=0,604), fatigue (Dfatigue=7 for CW and 6 for OCW; p=0,837) and recuperation (51 for CW and 61 for OCW; p=0,094).

    Well but ill: Lay perspectives on health among people with musculoskeletal disorders.2012Conference paper (Refereed)
    Abstract [en]

    This study departs from questions arisen in previous interview studies of health experiences with people with musculoskeletal disorders. During data collection about health experiences in this group a common comment to the question –“How do you experience your health”? was –“Well, but”…  This awoke our interest to further explore this topic. Thus the aim of present study was to investigate lay perspectives on health among people with musculoskeletal disorders. Semi-structured interviews were performed with 68 women and men with long term (>3 months) musculoskeletal disorders in neck, shoulder and/or low back. All informants were recruited as participants in previous studies on health experiences among people with MSDs (Wiitavaara et al., 2007a, b, 2008, 2009). The informants (39 women and 29 men), were 18-64 years old and most of them were occupationally active in a variety of different occupations. Analysis was performed using qualitative content analysis (Graneheim & Lundman, 2003). The lay perspective on health in this group of people with musculoskeletal problems was perceived as “Having resources and possibilities to lead the life one want”. This theme incorporated three main categories as the informants perceived health to include “A good enough physical and psychological functioning and balance”; “Freedom of action”; and “A positive state of emotion and an enriching social life”. The informants expressed a holistic view of health, encompassing physical, psychological, emotional, as well as social aspects, which can be interpreted from an action theoretical perspective (Nordenfelt, 2007).

    Background. Wound healing is a complex process, which requires adequate energy sources, proteins, and specific minerals and vitamins. If an individual is unable to get or to eat the nutrients required, the wound healing process might be disrupted. The aim of this study was to investigate food-related factors, meal patterns and meal quality in relation to nutritional status in elderly out-patients with leg ulcers. Methods. Nutritional status was assessed by use of the Mini Nutritional Assessment in 70 patients living in their own homes. Fifty-six of the patients recorded actual meals and snacks over four consecutive days. Meal patterns and meal quality were evaluated with the help of a qualitative classification system, the Food Based Concept for Classification of Eating Episodes. Results. Thirty-six patients were classified as well-nourished, 32 were at risk of malnutrition and two were malnourished. More patients in the risk group for malnutrition did not buy their own food, and usually ate alone. Incomplete Meals and Low Quality Snacks were the most common eating types. The patients at risk of malnutrition had significantly fewer prepared Complete Meals than the well-nourished patients. Conclusion. The results show a diet and meal quality which hardly meets the requirements for nutrients that are important in wound healing, especially for those patients assessed at risk of malnutrition.

    Background: The aim of this study was to evaluate whether qualitative meal-classification used as a dietary evaluation tool reflects eating frequency and key nutrients in the diet, and whether it could be used to monitor the dietary quality in elderly outpatients. The qualitative meal-classification tool used was based on eight food categories combined into four types of meals: Complete, Incomplete, Less Balanced and Vegetarian Meals, and three types of snacks: High-, Mixed- and Low-Quality Snacks. Methods: Dietary intake was assessed during 7 consecutive days by use of an estimated food record in nine elderly women with leg ulcers. Mean 7-day dietary intakes of energy, macronutrients, beta-carotene, retinol, vitamin E, ascorbic acid, calcium, iron, zinc and dietary fibre were calculated. Each eating event was classified in accordance with the food-based classification method. The frequency of the four types of meals and three types of snacks, their energy content and nutrients, and their contribution to total intake were subsequently analysed. Results: Meals were associated with a higher energy, vitamin and mineral content than were snacks. Complete Meals were the major source for beta-carotene and ascorbic acid. Intakes from Low-Quality Snacks were relatively high in energy fat and sucrose. Conclusion: The qualitative meal-classification tool was found to be simple to use and useful for reflecting meal order and the nutritional roles of different types of meals and snacks in the diet. This method is recommended for dietary monitoring.

    Abstract [en]

    The aim of the study was to describe the nutritional intake, meal patterns, physical activity and need for help in nine women living in their own homes and being treated for venous leg ulcers. Food habits were identified by use of interviews and food diaries completed by the women during a period of seven days. The intake of energy and nutrients from 304 eating events during seven days was calculated and meal patterns were evaluated using a qualitative system for meal classification. Physical activity and the degree of need were identified with the help of interviews. The intakes of energy and key nutrients for wound healing, such as protein, vitamin C and zinc, were not optimal according to the Swedish nutrition recommendations, although food habits were well organized. Most of the women had hardly any physical activities and the need of help and support varied, from daily visits to visits every second week.

    The purpose of this study was to examine mental health in relation to unemployment among newly arrived with post-secondary education. This was to get a deeper understanding about their experiences regarding unemployment despite their high education, the focus was on mental health and how they think their future looks like. The method used for the following study was an empirical qualitative method in which the author used the semi-structured interviews. Five participants were interviewed, four men and one woman between the ages 20-64. The results of this study found that newly arrived people with post-secondary education had poor mental health because of worries about the future. They also felt that the education they had received from their country was a waste in Sweden. These people also had a temporary residence permit which brought frustration because their future wasn’t secure. The conclusion of this study was that newly arrived individuals with post-secondary education wanted Psychological help from the society so they could start working.
