    Abstract [en]

    The aim of this study was to examine the experience, of four different actors’ with diverse occupational roles, of the existing rehabilitation process in the organization, with its limitations and possibilities. A further aim was to examine the preventive work and hopefully contribute with some practical solutions to counteract sick leave within the organization. Four mini focus group interviews with two to four people were carried out with two fictitious cases as a base for discussion. Data was analyzed by an inductive thematic analysis and resulted in four thematic areas; Tools to improve the health work, Efforts to avoid sick leave, Health work limitations and The manager’s role in health work. The study showed that the participants considered that the rehabilitation process was a good tool that simplifies the rehabilitation work, but it needs further development. The results also showed that the participants had many suggestions of how to improve the physical and psychosocial work environment.

    Milk consumption and growth of children in the Kilimanjaro region in rural Tanzania.: An ethnographic research done through quantitative and qualitative methodes.2013Independent thesis Basic level (degree of Bachelor), 10 credits / 15 HE creditsStudent thesis
    Abstract [en]

    The aim of this study was to investigate milk consumption among children in a village in a developing country where access to milk during childhood may be limited. Another aim was to see if measurable information could demonstrate growth among children who drank milk. The data was collected through anthropometric measurements on 66 boys and girls (aged 5-6). Nutritional data from a selected group of parents of the participating children were done through 24-hour-recall, questions about meal arrangement, availability and attitudes about milk were asked.

    Result: Five-year-old girls who drank milk at home and in school were significantly taller compared to girls (5 yr) who did not drink milk, as well as taller than the girls (5 yrs) who drank milk only at home. Arm muscle area, and arm circumference was also significantly larger among the girls (5 yr) who drank milk. No significant differences were shown among the boys. Food consumption included more nutrients among children who drank milk, which might contribute to the growth effect among the girls (5 yrs). Consumption of milk was due to economic situations, access to school-milk or owning a cow.

    Conclusions: Some significant differences among 5-year-old girls were shown. Poverty was the main reason for not drinking milk, as well as lack of access to milk.

    Abstract [en]

    Background: Pressure sores occur as a complication of care, treatment and disease where these can occur already after 60 minutes. The incidence of pressure ulcers in 2016 at 13.4% averaged over the whole of Sweden, as compared to 2015 is a reduction of only 0.2%.

    Aim: To describe the nursing care nurse can take in their work to prevent pressure sores and to report the selection method used in the selected articles.

    Method: Literature study with descriptive design that was based on thirteen articles from the databases CINAHL and Pubmed.

    Findings: The result of the above literature led four different themes: 1) Risk Assessment: By making a risk assessment of patients within 24 hours of hospitalization the risk of pressure sores was reduced. 2) Location Changes: Location Changes to the patients every two hours showed that the incidence of pressure ulcers decreased. Further, it has been shown that positions lying at 30 degrees is the method that provides the best output. 3) Pressure relief: Use of different pressure relieving agents have shown to reduce pressure sores in patients. The best method seen from differences in the control and intervention groups for pressure relief in this study is the use of a silicon border. 4) Collection of preventive care measures: The use of different protocols with several different care methods to reduce the occurrence of pressure sores have shown the best effect. Methodological aspect: 11 of the articles had a clear description of the sampling method, with the remaining two items missing description of how the selection had been made.

    Conclusion: The present literature review has shown that the above themes can be used by nurses in their daily work to reduce the risk of pressure ulcers in patients.

    Abstract [en]

    Aim: The aim of the study was to describe the district nurse's and nurse's reflections on work in telephonenursing in primary care. Background: Telephonenursing today represents a major part of nurses' work in primary care. The classical patient visit has in part been replaced by telephonenursing, where nurses are expected to have broad medical and social skills, as well as educational skills. Working in telephonenursing can be complicated, challenging and stimulating. Method: The study used a qualitative approach with descriptive design. Nine telephone nurses from primary care participated in the study and were interviewed January-May 2017. Result: The results in this study showed that telephonenursing was experienced as stressful, demanding and complex with limited control capabilities. Timepressure at work, resource shortage in health care and disturbed work environment felt as strenuous circumstances that could lead to stress. Telephonenursing was also described as fun, varied, satisfying, evolving and problem solving was a prominent feature. Good conversations with patients, as well as support at work were described as important factors. Conclusion: The nurses in this study experienced that work in telephonenursing was complexed with high demands and strains. However, it was also a fun, varied and satisfying work. Continuous education and slightly reduced demands can lead to an improved work situation, where the risk of impaired health for nurses can be reduced and patient safety can be strengthened.

    Syftet med denna studie var att undersöka samband mellan bukaorta aneurysm hos 65-åriga män och riskfaktorer såsom rökning, BMI>25, kosttillskott, hypertoni, hyperlipidemi, ett stillasittande yrke samt hereditet. Kvantitativ ansats med deskriptiv och korrelativ design användes, 3854 65-åriga män från två årskullar som screenats för AAA ingick och uppgifter hämtades från ett dataregister. Resultat: Antalet personer med bukaorta aneurysm var 2,4 %, 65 % var/hade varit rökare, 67 % hade övervikt, 49 % stillasittande arbete, 41 % hypertoni, 25 % hyperlipidemi, 17 % åt kosttillskott och 4 % hade hereditet. Rökning, hypertoni och hyperlipidemi visade signifikant samband med och ökade risken för att utveckla AAA. Störst riskfaktor var rökning. Övriga riskfaktorer visade inte signifikant betydelse. Riskfaktorerna tillsammans förklarade variationen i AAA med 5 %. Konklusion: Tidigare kända riskfaktorers betydelse bekräftades för uppkomst av AAA. För män med AAA ses ett behov av hälsoförebyggande insatser. Sjuksköterskan har ansvar för och kunskaper om preventiva åtgärder, kan och bör därför användas som stöd till män med nyupptäckt AAA.

    Abstract [sv]

    Syfte: Syftet med föreliggande litteraturstudie var att beskriva vad som orsakar fallolyckor hos äldre inom vårdhem, beskriva preventionsåtgärder samt att granska de urvalsmetoder som de granskade artiklarna använde sig av.Metod: Artiklar söktes i databaserna PubMed, Medline samt Cinahl med publiceringsbegränsning mellan år 2008-2013. Totalt 16 artiklar inkluderades.                 Resultat: Föreliggande litteraturstudies resultat delades upp i två huvudkategorier med sex underrubriker varav fyra berörde orsaker till fallolyckor och två behandlade preventionsåtgärder. 1) Individuella faktorer; flera individuella faktorer såsom ålder, ostabil gång samt medicinsk bakgrund visade sig orsaka fallolyckor hos äldre boende på vårdhem. 2) Fysisk vårdmiljö; många aspekter av vårdhemmets miljö orsakade fallolyckor hos äldre boende på vårdhem, exempel på dessa aspekter var rummens utformning samt inredning. 3) Hjälpmedel; ett samband påvisades mellan fallolyckor och hjälpmedlet rullstol där antalet fallolyckor ökade vid användandet av rullstolar. Användandet av fysiska begränsningsåtgärder visade sig ha olika effekt på antalet fallolyckor. 4) Läkemedel; medicinering med centralverkande läkemedel orsakade en ökad risk att drabbas av fallolyckor medan icke-centralverkande läkemedel inte visades orsaka fallolyckor. 5) Fysiska preventionsåtgärder; interventionsprogram med fysiska preventionsåtgärder visade sig ha olika effekt på antalet fallolyckor. Det visade sig att olika undergrupper påverkades olika av preventionsåtgärder. 6) Preventionsåtgärder genom vårdpersonal; preventionsåtgärder genom vårdpersonal visade sig inte ha någon effekt på antalet fallolyckor. Metodologisk aspekt gällande urvalsmetod; av de granskade artiklarna framgick en tydlig beskrivning av urvalsmetod i åtta artiklar, i två artiklar beskrevs urvalsmetoden men ej utförligt och sex artiklar saknade helt beskrivning av urvalsmetod.Slutsats: Fallolyckor är ett världsomspännande folkhälsoproblem och förekomsten är stor hos äldre boende på vårdhem. Föreliggande litteraturstudie visade att ett flertal orsaker till fallolyckor hos äldre boende på vårdhem och fallolyckspreventionsåtgärder var belagt i vetenskapliga studier men att mer forskning krävs för att stärka bevisvärdet samt att förbättra preventionsarbetet. För att minska antalet fallolyckor hos äldre boende på vårdhem krävs det att sjuksköterskan ser till individen och arbetar förebyggande samt utvärderar de handlingar som utförs.

    Problemformulering: Tidigare studier tyder på att den svenska arbetsmarknaden är könssegregerad samt att det förekommer en könssegregering av arbetsuppgifter inom samma yrkestitel. Detta innebär att män och kvinnor utsätts för olika typer av mentala och fysiska belastningar. Eftersom arbetshälsovetenskapen delvis har till syfte att studera orsaker till arbetsrelaterade belastningar samt hur arbete kan organiseras för att minska belastningar inom arbetet är jämställdheten på arbetsmarknaden relevant ur ett arbetshälsovetenskapligt perspektiv. Syfte: Syftet med studien var att undersöka ambulanssjuksköterskors uppfattningar av jämställdhet inom yrket. Metod: Kvalitativ studie med fenomenografi som ansats. Semistrukturerade intervjuer med 10 ambulanssjuksköterskor. Huvudresultat: Intervjumaterialet utmynnade i tre kategorier relaterat till jämställdhet inom yrket: samma förutsättningar oavsett kön, kön inverkar på bemötande från patienter och anhöriga samt förändringar över tid. Resultatet tyder på att ambulanssjuksköterskorna i studien uppfattar yrket som jämställt utifrån att de har samma möjligheter till utbildning, arbetar under samma förutsättningar samt att de har samma rättigheter. Vidare uppfattade ambulanssjuksköterskorna i studien att manliga och kvinnliga ambulanssjuksköterskor behandlas olika från patienter och anhöriga. Ambulanssjuksköterskorna beskrev även uppfattningar om att jämställdheten inom yrket har förändrats över tid då yrket tidigare varit mansdominerat. Slutsats: Ambulanssjuksköterskorna uppfattade att män och kvinnor verkar under samma förutsättningar inom yrket. En tänkbar förklaring till detta kan vara den tydliga strukturen på arbetsplatsen. Vidare uppfattade ambulanssjuksköterskorna att kön inverkar på bemötande från patienter och anhöriga samt att jämställdheten inom yrket har förändrats över tid, då yrket tidigare varit mansdominerat. Framtida forskning bör därmed fokusera på strukturens betydelse för jämställdhet inom arbetslivet samt vilken betydelse genus kan komma att ha på arbetsmiljön.

    ABSTRACT

    In the year 2003 and 2005, several legislative changes were made to same-sex couples advantage. Among other things, lesbian couples were given the right to be tested as adoptive parents and the possibility of assisted reproduction through the county councils. With such changes in law, society and family constellations are evolving today with more forms than the existing nuclear family. Similarly, the choice of approaches to forming a family in a same-sex relationship increases, two alternatives outside the county Council's assisted conception are; Insemination by clinic in another country or home insemination with private donated sperm. When insemination in same-sex relationship occurs at home or by clinic in another country, subsequent paternity investigations and Related adoption are required for the non-biological mother to be considered as legal parent, an investigation requested by the District Court and operated by Social Service in each municipality. With the possibility of same-sex couples to be tested as adoptive parents also follows expectations about the investigation procedure and its guidelines as a partner in the development. The aim of this study is to try to create a synthesized knowledge about the investigation process of paternity investigation and Related adoption, when it occurs in same-sex relationship. This by jointly trying to understand the experiences of women and Social workers ' in the investigation process and its content. The study is based on qualitative interviews with both women who have undergone the process and social secretaries who dealt with these cases. The result is a dominant shared experience of unclear guidelines where there is a demand for nuanced legislation and updated dossiers.

    Syftet med denna studie var att öka förståelsen för hedersproblematik inom ramen för socialt arbete samt hur problemet kan begripas av professionella som kan komma att stöta på hedersrelaterat våld och förtryck i sitt yrke. Studien baserade sig på en kvalitativ metod, en kunskapsöversikt som hade en hermeneutisk ansats för att försöka ökan kunskapen för hedersproblematik. Med 11 primärdokument urskiljde vi tre teman; kvinnans underordnade ställning, kultur och generation, lojalitet och makt. Resultaten visade att det finns olika sätt att förklara hedersrelaterat våld och mord, men vi fann överensstämmelser; att flickans oskuld måste vara icke ifrågasatt, om bara så ett rykte om motsatsen förekom fanns det en direkt fara för hennes liv. Kultur var en omdiskuterad förklaring till fenomenet, där resultaten visade att de kulturella normerna och synsätten var olika i en kultur där det praktiserades heder i jämförelse med en kultur utan. Dessa resultat analyserades sedan socialkonstruktionistiskt.  Vår slutsats av denna studie är att hedersrelaterat våld konstrueras beroende på kön, kultur, generation, lojalitet, makt och det samhälle man befinner sig i. Beroende på vem man är i vilken kontext har hedersrelaterat våld och mord olika betydelse och förstås på olika sätt.

    Abstract [en]

    This paper focuses on different forms of intimacy in later life in late modern Sweden and their consequences for well-being. The results are based both on qualitative interviews with 28 Swedes (63–91 years), living in new relationships initiated after the age of 60 (marriage, cohabitation, LAT) or currently dating; and on a quantitative survey to 60–90 year old Swedes (n=1225; response rate 42%). The results show: A great diversity in relationship careers over the life course; a preference for LAT (70%) and cohabitation (26%) in new unions established 60+; strong support from adult children for parents’ new LAT (86%) and cohabitation (76%) relationships, but less support for marriage (50%); that new relationships are as important for life-satisfaction as long-lasting ones; after subjective health, a partner is the second most important explanation for life-satisfaction, more important than having children; LAT was the union form that added most to life-satisfaction for men while marriage was the only form that significantly added to the life-satisfaction of women. Results were the same for newly established relationships and older relationships. Results will be discussed in relation to the shift from marriage to divorce culture (Hackstaff), the transformation of intimacy (Giddens), potentials of the third age (Laslett), and gender socialization, but also in terms of how the results can be interpreted in a context of Swedish welfare-state supported individualism where a partner provides both autonomy in relation to adult children and social integration.

    Changing Union Forms Among Older People in Late Modern Sweden2015In: Aging Families/Changing Families, 2015Conference paper (Refereed)
    Abstract [en]

    This paper focuses on union form in cross-gender relationships in later life, against the background of the transformation of intimacy in late modernity. Results are based on a survey to 60–90 year old Swedes (n=1225; response rate 42%) and European census data. Sweden seems to be the only country where there are more divorced than widowed people in this age group. Almost 1/3 of Swedes, aged 60-90, categorized as “singles” by official Swedish census data on civil status, are in fact living as LATs or cohabitants. In new romantic relationships initiated 60+ the dominant union form is LAT (70%) followed by cohabitation (26%), while marriage is rare (4%). Less than 2 in 10 singles think that is important to be married – and among marrieds less than 8 in 10. Relationship history data shows that although half of the respondents have been married only once, one third (33%) have had 2+ cohabiting unions (marital/non-marital), half (46%) 2+ established relationships, and a majority (66%) 3+ sexual partners. The results indicate that the transformation of intimacy includes older Swedes. Discussion: Should we see older people as a vanguard in the exploration of late modern intimacy, rather than carriers of cultural lag?

    Social gerontology and family sociology about intimacy in old age has almost exclusively focused on institutionalized life-long marriages and little research has focused on re-partnering and the forms and meaning of this diversity of new intimate relationships in later life. Framed by Giddens’ transformation of intimacy and Laslett’s theory of the third age, as well as changing social and demographical conditions, this paper will focus on how different forms of new intimate relationships are formative for the experi-ence of ageing.The strategical sampel was diversified according to differ-ent forms of new intimate relationships: dating singles, married, cohabiting and living apart together. Qualitative interviews was used with 28 Swedes, 63–91 years, who have established a new intimate heterosexual relation-ship after 60 or who are dating singles. The results showed the significance of new intimate relations for social integration and for the experience of old age as the “crown of life”. The meaning of new intimate relations in differ-ent intimacy forms are discussed in relation to everyday life in old age, the relational history and relational careers and future perspectives of informal support structures by the relation.

    According to contemporary family theory late modern relationships are guided less be external norms and more by internal negotiation between relatively equal partners. The purpose of this paper is to study initiation and development of new intimate relationships in later life, with a focus on negotiation and change, based on relationship histories with older Swedes (n=28), who are currently in new late in life cross-gender relationships initiated 60+, or currently dating singles. The results show a paradox: relationship ideals often seem unchangeable in prospect, but actual relationship arrangements appear open and changeable, when described in retrospect. LATs recall having been determined not to initiate any new relationships, cohabitants to retain their own home, remarried informants never to marry again. The analysis focuses the negotiations leading to relationship change. We discuss and question prevailing implicit assumptions about older people’s relationships as non-negotiated and unchangeable.

Based on theories about the shift from marriage to divorce culture (Hackstaff), the transformation of intimacy (Giddens) and potentials of the third age (Laslett), this paper focuses on forms of intimacy in later life in late modern Sweden. The results arebased partly on qualitative interviews, including relationship biographies, with 28 Swedes (63–91 years) living in new heterosexual relationships initiated after the age of 60 (marriage, cohabitation, LAT) and dating singles. And partly on a quantitative survey to 60–90 year old Swedes (n=1225). The results show: A great diversity in relationship careers; a preference of LAT and cohabitation (non-­‐visible in official statistics); the importance of intimate relationships for well-­‐being and that new relationships are as important for life-­‐satisfaction as long-­‐lasting ones; that intimate relationships are more important than children for well-­‐being. A central analytical finding was the importance of time as an organizing frame for new intimate relationships in later life: the paradox of having a lot of free time in the third age, but a restricted life time left. Results will be discussed in relation to the theorizing frames mentioned above, especially new practices for intimacy in a culture of divorce. Cohort replacement is only one explanation for changing practices and the period effect seems to be as important.

    Age homo/heterogamy in romantic unions is given little attention in current gerontological research. Still partner age-differences are important for men’s and women’s relationship opportunities. This paper studies actual partner-age (age-difference) in cross-gendered unions, and ideal partner-age both for singles and respondents in unions. The study was carried out by a survey to 60–90 year old Swedes, currently either singles or in a cross-gender relationship (married, cohabiting, LAT) (n=1225). Unions follow a traditionally gendered age structure: 56% of men, but only 16% of women have a younger partner. This pattern is more pronounced: in first unions (p<.001) and unions initiated before 1970 (p<.01). For respondents in unions actual partner-age showed no significant correlation with union form or urbanity (modernity-hypothesis), and not with education or income (power-hypothesis). For respondents in unions Ideal Partner-Age correlates strongly (p<.001; R2=0,76) with actual partner-age (ideal slightly younger). Among single respondents, almost all (92%) single men and half of the single women (47%) prefer a younger partner (8,9 years younger on average for men; 2,2 for women). The proportion preferring a younger partner increases by age, leading to increasingly incompatible age ideals. Results will be discussed in relation to life-course theory; gender and power; the deinstitutionalization hypothesis.

    Sexual activity and norms among older Swedes: a life course perspective2014Conference paper (Refereed)
    Abstract [en]

    It has been argued that in late modernity sex has escaped its reproductive cage and people form pure relationships, based on mutual satisfaction. Ironically, although older people are per definition non-reproductive, they have often been neglected in studies on sexuality. In this paper we present results from 1) a qualitative interview study with a strategic sample of 28 63–91 year old Swedes currently dating or in a heterosexual relationship (married, cohabiting, LAT) initiated 60+ and 2) a quantitative survey including answers from 1225 60–90 year old Swedes. The interviews revealed a clear normative change, from a cultural context that condemned extra-marital sex in young adulthood to a context which encourages sexual relationships but not marriage in later life. All had experienced the sexual liberation of the 1960s, and today, these liberal attitudes seem to encompass later life. Today, an active sex-life was regarded as important for a good relationship and sexual attraction was seen as a precondition for new relationships. Many informants had interpreted sexual decline in former relationships as “natural” and age-related, but re-discovered sexuality with their new partner. In the survey, a majority (93 %) had had their sexual debut before marriage (despite the restrictive norms). Half or the respondents have had ≥ 4 more sexual partners, and one in five ≥ 10. Sexual activity correlated negatively with relationship length. It has often been argued in gerontology that sexual values and practices will become more liberal in the future by cohort replacement. Our data indicates that “the future might already be here”.

    Introduction: Social gerontology about intimacy in old age has almost exclusively focused on institutionalized life-long marriages. However, mobility in and out of intimate relationships has become more common in late modern societies also in later life. In this paper the research questions are: What characterizes the formation of new intimate relationships in later life? Are there any specific, more or less universal, conditions that separate them from relationships in earlier life phases?

    Method: Qualitative interviews were used with a strategical sample of 28 6-?91 year old Swedes, who have established a new intimate heterosexual relationship after the age of 60 or who are dating.

    Results: The results showed time as constituting a central structuring condition for new intimate relationships in later life. In the results three aspects of time: Available free time, Lived time and Remaining time ? which all have a constituting and formative power on new late in life relationships are discussed in relation to theories of late modernity and the Third Age and in relation to changing demographical conditions.

    Conclusion: The concluding discussion will be about the time theoretical frame as a model for understanding experiences of ageing.

    The purpose of this article is to study the importance of intimate relationships as protection from loneliness in later life. We base ourselves on a survey to Swedes aged 60–90 (n=1 225) focusing on intimate relationships. The analysis considers neglected issues in ageing research on loneliness: the importance of union form, the importance of looking at relationship dissolution in terms of both widowhood and divorce, and the importance of new late life unions (a gains perspective). We use two theoretical perspectives: the discrepancy model (realities vs. ideals), and the protection hypothesis, where the partner is generally the first and most important source of support in everyday life. The results show that a partner protects against loneliness and that union form matters: marriage provides the best protection, followed by cohabitation and Living Apart Together (LAT). Feelings of loneliness decrease over time following a union dissolution – and, for men, more rapidly after separation than widowhood. The more the ideal union form differs from one’s actual union form, the more common are feelings of loneliness. Initiating a new relationship after a union dissolution protects against loneliness. The article discusses the importance of using union form instead of civil status as relationship indicators in studies of older people in late modern Sweden, and of including separation/divorce as indicators of union dissolution besides widowhood. It also stresses the importance of looking at later life not only from a loss – but also from a gains – perspective.

    Aim Little is known about the health-related quality of life (HRQoL) of children with lower urinary tract dysfunction (LUTD) and chronic kidney disease (CKD). We investigated LUTD and other possible predictors of impaired HRQoL in children with conservatively treated moderate to severe CKD or with a kidney transplant.

    Methods All 64 children with CKD or a kidney transplant treated at Karolinska University Hospital, Stockholm, Sweden, between June 2011 and December 2012 were approached and 59 children aged 8-18 were enrolled in the study. Lower urinary tract function was evaluated with voiding history, frequency and volume chart, uroflowmetry and post void ultrasound measurements. Self-reported HRQoL was assessed with validated generic instruments.

    Results The HRQoL of the study cohort was as good as the general paediatric population, apart from the physical and psychological well-being dimensions, and was no different to children with other chronic conditions. Urinary incontinence, but not LUTD in general, was associated with impaired HRQoL, as was having a kidney transplant and being female in some dimensions.

    Conclusion LUTD was common in children with CKD or a kidney transplant but did not affect their general HRQoL. Predictors of impaired HRQoL included incontinence, having had a kidney transplant and being female.

    Bakgrund:

    Patienter med höftfraktur blir allt fler i takt med att medelåldern ökar. Det är en stor och resurskrävande patientgrupp. De har under en längre tid varit underprioriterade vilket har resulterat i långa väntetider till operation.

    Syfte:

    Syftet var att beskriva om det fanns några fördelar att arbeta med höftspår vid höftfrakturer samt att beskriva urvalet av de använda vetenskapliga artiklarna.

    Metod:

    Deskriptiv litteraturstudie med tretton vetenskapliga artiklar med kvalitativ och kvantitativ ansats. Likheter och olikheter i artiklarnas resultat har kategoriserats och sammanställts till fem olika teman.

    Huvudresultat:

    Resultatet visade att det fanns fördelar med att arbeta med omhändertagande via höftspår, detta resulterade i kortare väntetid till operation samt färre komplikationer. Dessa patienter som omhändertogs via höftspår hade även en kortare vårdtid. Risken för mortalitet påverkades inte nämnvärt vid höftspår kontra konventionell vård vid höftfraktur.

    Slutsats:

    I kategorien förkortad tid till operation samt kortare vårdtid kan man se stora positiva skillnader efter införandet av höftspår, i minskade postoperativa komplikationer en viss förbättring. I kategorin minskad risk för dödlighet ses ingen statistisktsignifikant skillnad. Riktlinjerna kring detta höftspår har påverkat omvårdnaden i en positiv riktning. Dessa patienter omhändertas nu med en snabbare och bättre rutin. Det har inte upptäckts några nackdelar med att arbeta med höftspår.

    Bakgrund Stroke tillhör en av våra största folksjukdomar och förblir en av de sjukdomar som kräver de högsta dödsantalen och ger allvarliga följder med omfattande funktionsnedsättningar. Följderna efter en stroke varierar beroende av vart och med vilken omfattning skadan orsakat, vilket ger en varierande symptombild med olika grader av funktionsnedsättningar. Detta gör omvårdnaden av en strokepatient till komplex som ställer höga krav på en individuellt anpassad vårdplan med anpassade professioner som omger patienten.

     

    Syftet med denna litteraturstudie var att undersöka sjuksköterskans erfarenheter av att vårda patienter med en strokediagnos. 

     

    Metod En beskrivande litteraturstudie som baserats på resultatet av 10 kvalitativa och 3 kvantitativa artiklar.

     

    Huvudresultat Det framkommer att sjuksköterskan har en viktig uppgift i att arbeta kliniskt och nära patienten men att det på grund av brist på resurser och utbildning är svårt att ge en tillfredställande omvårdnad. Sjuksköterskans position bland andra professioner upplevs inte heller som självklar och behöver vara väldefinierad för att kunna säkerställa att patientens vård är av hög kvalitet. Det framkommer även att sjuksköterskan i sin omvårdnad behöver involvera patienten mer i kommunikationen genom att lära känna personen bakom patienten för att på så vis förbättra återhämtningen efter en stroke.

     

    Slutsats Sjuksköterskan utgör en nyckelroll vid omvårdnaden av strokepatienten. Studier visar dock att det saknas kunskap, utbildning och förståelse från både organisation och sjuksköterskor vilket på flera sätt sänker patientens vårdkvalitet.

    Background: Basic hygiene is an important measure to prevent healthcare associated infections, save patients’ lives, and reduce economic costs for health care. Healthcare associated infections are a threat to patient safety. In community care persons who are the care takers are a risk group and nurses must make sure that basic hygiene procedures are followed.

    Method: Literature studies with descriptive design, article search in databases Cinahl and PubMed. The results found are based on twelve chosen articles.

    Aim: To describe factors related to adherence to basic hygiene and how nurses in community care can help to improve compliance with these procedures.

    Results: In the nursing staff and leadership in health care interest in and the understanding of basic hygiene is described as being of most importance. Lack of knowledge of the meaning of patient care cleanliness, negative attitudes and non-existing availability of necessary tools and heavy workload had a negative impact. Intensified education and making sure that equipment is available as well as the application of standardized methods, were found to be prerequisite for adherence to basic hygiene routines.

    Conclusion: Nurses can contribute to improved adherence to basic hygiene routines by promoting education, positive attitudes, a positive adaption of the physical environment, strategic structuring of the workload, and standardized methods, applying basic hygiene routines, being part of and promoting increased interaction in nursing care. Suggestions for future studies is to implement web-based courses, updating courses and training in basic hygiene routines in the workplace. That may increase knowledge, understanding and awareness of the application of basic hygiene routines hence leading to improved nursing care and enhancing patient safety.

    Background: Every year approximately 2000 people in Sweden fall ill to some kind of psychotic disorder. Psychotic disorders are often associated with prejudice and stigma. Research has shown a connection between the expected external stigma, internal stigma and the recovery of patients with a psychotic disorder. It has also been described that patients with a psychotic disorder has received insufficient care due to their psychiatric illness.    

    Aim: The aim of this study was to describe how nurses experience the encounter with patients that are suffering from a psychotic disorder within the somatic inpatient and outpatient care.

    Method: A descriptive literature study based on 5 qualitative and 5 quantitative scientific articles. The databases that was used to obtain articles was PubMed and Cinahl.   

    Main result: It appeared that there is a substantial lack of knowledge regarding psychotic disorders within the somatic care. Nurses often experienced feelings of fear, tension and insecurity, which made the encounter with these patients problematic. Limitations such as time constraints contributed to unfavourable conditions in the care of patients with a psychotic disorder.   

    Conclusion: Stigma, lack of knowledge and deficiencies in the care environment could make the care that patients with a psychotic disorder receive inadequate. Therefore, it is important that nurses in the somatic inpatient and outpatient care become aware of their own preconceptions and attitudes towards these patients, and that nurses get access to additional knowledge on this topic. The somatic units should also try to make improvements in the care environment.

    This thesis is a qualitative study with the intent to examine how a number of clients and counselors describe their experiences and impressions of the importance of high-level trust vs. low-level trust relationships between the client and the counselor. We have separately interviewed two clients and one counselor from an addiction treatment center (HVB-home). We found several similarities in the clients´ stories. Both had, for example, experienced weak attachment during their childhood and their addictions were about ridding anxiety. Both clients have had prior experiences of low-level trust relationship with counselor which has led to a negative impact on their treatment. The counselor we interviewed believed that a client’s earlier attachment experiences can influence whether or not a new relationship between a client and a counselor will be successful. That which we have found most interesting in the interviews is that all three interviewees agree that a high-level trust relationship between the client and the counselor is of high importance for the outcome of the treatment.

It is the nurses in the ambulance crew who assess whether pain relief is required urgently or not during transport to hospitals.   Objective: The purpose of this study was to study patient´s experiences of intranasal (i.n) pain relief compared to intravenous (i.v) pain relief within the ambulance service in Stockholm area.    Method: The study is an empirical study with a descriptive design.   Main results: The study showed that the only significant difference in the area where the experience of anxiety and/or fear was measured in connection with i. n versus i. v. pain relief method. It was more that experienced a negative aspect in the form of anxiety and or fear in the group that received in n pain relief. No other significant differences in the experience of pain relief, satisfaction of neither the ring's power, the way to obtain the medicine or experience side effects was demonstrated. A comparison of the reported showed that the most common side effect was nausea, and it was most common in the group that received in n pain relief.   Conclusion: Based on this result, one can question whether the mode of administration is crucial for the effect and experience of positive or negative aspects of pain management.

    Abstract [en]

    Aim: The aim of this literature review was to describe how the nurse can support overweight children and their families and to describe what support these children and their families request from the nurse in order to make a healthy change. Furthermore the intent is to review the included articles quality regarding the data collection method.

    Method: A systematic research in the databases Cinahl and Academic Search Elite was used to find the articles. The systematic research lead to 11 articles that was included in this study.

    Result: Children suffering from overweight and their parents wish to get an increased support from the health care through in example individual programs for the entire family. The nurse can use a neutral language and the growth chart in order to make the families observe the overweight of the children. Another good method for the nurse to use is motivational interviewing. Different intervention programs have been produced to support overweight children and their parents and are a good way to highlight the living habits and to make an individual plan for the families. The data collection method is explicitly described in all of the 11 included articles.

    Conclusion: The present literature review shows that overweight children and their families want more support from nurses and healthcare. Nurses need more education to be able to pay attention to the kids and parents on the issue. By developing and strengthening the procedures and tools that have been developed, the nurse may be strengthened in their professional role in efforts to support overweight children and their parents.

    Aim: Examine preschool teacher’s perceptions about pedagogic meal.

    Design: The survey included an inductive approach, intensity sampling, semi-structured interviews and a qualitative content analysis.

    Results: The result was presented on the basis of four main categories; the first, The preschool teacher´s view of the pedagogic meal. The second, Factors in the environment that affect the educational meal with the subcategories Room and group size, How food is served, Space and time for the meal, Interruptions during meals. Subsequently, the category Socialization during the meal with two subcategories, Norms and rules & Learning strategies affect children's habits. Finally, the category Preschool teachers’ knowledge, background and development with the subcategories Learning strategies affect children's habits & skills development is needed.

    Conclusions: Further research is proposed to be quantitative research, a survey to test what emerged in the results of this study. This would provide a basis for creating guidelines and policies for how a pedagogical meal should be shaped more concretely.

    Aim: to examine the effects on the lives of people who are related to someone with dementia.

    Method: A literature review with descriptive design. The study was based on 13 articles collected in the databases PubMed and Cinahl.

    Result: The results showed a marked change in the lives of those related to a person diagnosed with dementia, and the life situation was affected in multiple ways. The type of relationship the relative and the ill person had before the disease could in some instances affect how the relative experienced the situation. The results demonstrated emotions like isolation, anger, powerlessness, as well as loss and guilt. In dealing with the role as caregiver factors like having a positive attitude and a good support system was mentioned. Very often the relatives would maintain their roles as caregivers as long as possible, but when the situation became untenable, the move to assisted living was inevitable.

    Conclusion: The results of the study showed that the overall impact on the lives of relatives of people diagnosed with dementia was negative. There is some research on how the lives of relatives of people with dementia are affected, but in order for nurses to give the best support possible, there is a need for even more research and publications on this topic.

    Bakgrund: Diabetes är en av de sjukdomar som är mest växande globalt. I Sverige är det över 400 000 människor som lever med diabetes. Diabetessjukdomen finns i olika former och de vanligaste formerna är diabetes typ 1 och diabetes typ 2. Risken att bli drabbad av den sjukdomen ökar vid fetma, tobaks- och alkoholkonsumtion. Därför kan det bli nödvändigt att genomföra livsstilsförändring för att förbättra den mentala och fysiska hälsan hos personer som lever med diabetes typ 2.

     

    Syfte: Syftet med denna studie var att beskriva hur personer med diabetes typ 2 upplever att genomgå livsstilsförändringar till följd av sjukdomen. Syftet var också att beskriva datainsamlingsmetoderna i de inkluderade artiklarna i resultatet.

     

    Metod: En litteraturstudie med en deskriptiv design som baserades på 14 kvalitativa vetenskapliga artiklar.

     

    Huvudresultat: Resultatet för denna litteraturstudie baserades på möjligheter och svårigheter som påverkade upplevelsen av att genomgå livsstilsförändringar hos personer med diabetes typ 2. Utifrån möjligheter har fyra subteman identifierat: Kost och motion som motivationsfaktorer, erhållen kunskap, betydelsen av stöd från omgivningen och att ta personligt ansvar. I huvudteman svårigheter presenterades dessa subteman: Kost- och motions påverkan på det sociala och vardagliga livet, brist på kunskap samt krav och inre kamp.

     

    Slutsats: Flera personer fick blandade känslor utifrån upplevda möjligheter och svårigheter som de har stött på under livsstilsförändringen. Det blir viktigt att sjukvårdspersonal får en bättre helhetsförståelse kring patienters olika upplevelser av att genomgå en livsstilsförändring för att kunna optimera en personcentrerad vård, egenvårdsinsatser och tillgodose patienters egenvårdsbehov.

    Abstract [en]

    Background: Every year, about 90 000 people in Sweden die and most of them are in need of palliative care. The individual should as far as possible be able to decide if he or she wants to die at home or in hospice care. Historically, palliative care was meant to treat people with cancer but has gradually evolved to treat several types of disorders.

    Purpose: The purpose of this study was to describe how adults perceive palliative care in their home, as well as to study the selected articles data collection method.

    Method: The present study has a descriptive design. The study results are based on eleven scientific articles, ten with qualitative approach, and one with a mixed approach. All articles were reviewed by both authors and color marked depending on the content. The articles were identified through databases CINAHL and Pubmed.

    Main Results: The palliative care at home was perceived largely as positive among patients. It was highly valued having their relatives close and to reside in their familiar home environment at the end of life. It also emerged that the palliative patients were concerned about being a burden to their families. By having paramedics in their home as well as support from relatives they were given the opportunity to be able to handle everyday activities on their own, which turned out to generate quality of life. Dignity of the palliative patients was described by the concepts of integrity, well-being and self-determination. Good communication and continuity was seen as important factors for the patients.

    Conclusion: Palliative patients who received care at home looked largely positive about the care they received. They felt that the cooperation between health care and their families was essential for safe care in their home. These patients' desire to be cared for and die at home should be a priority as it will lead to increased quality of life. Results of this study gives a clear indication of development areas in palliative care.

    Background: A functional integration between the jaw and neck regions during purposive jaw movements is well described in adults, but there is a lack of knowledge of such integration during jaw function in children.; Objectives: To determine the movement integration between the jaw and neck during jaw motor tasks in 6-year-olds, whether there is a difference between children and adults.; Methods: Jaw and neck movements were recorded with an optoelectronic 3D system in 25 healthy 6-year-olds (12 girls, 13 boys) and 24 healthy adults (12 women, 12 men) during paced jaw opening-closing and self-paced gum chewing. Jaw and neck movement amplitudes, intra-individual variation in movement amplitude, ratio between neck-jaw movement amplitudes, and movement cycle time were analysed. Differences between children and adults were evaluated with Mann-Whitney U test for independent samples.; Results: Compared to adults, 6-year old children showed larger neck movement amplitudes (P=0.008) during chewing, higher intra-individual variability in amplitudes of jaw (P=0.008) and neck (P=0.001) movements, higher ratio between neck-jaw movement amplitudes for jaw opening-closing (P=0.026) and chewing (P=0.003), and longer jaw movement cycle time (P≤0.0001) during the jaw opening-closing task.; Conclusion: Despite integrated jaw-neck movements in 6-year old children, the movement pattern differs from that of adults and may be interpreted as an immature programming of jaw-neck motor behavior. The well-integrated movements observed in adults most likely develop over years, perhaps into adolescence, and needs further research including well controlled longitudinal studies to map this development in order to provide appropriate age-related clinical treatment for functional disorders.     “Waiting room dilemmas- an ethnographical study of interactions and dilemmas in social welfare waiting rooms” 

    The social services activity is a balance between service and control. Uniting these parts is a challenge, and this study’s main intention is about how various social offices relates to this in their work with peoples waiting room experiences. The purpose of the study is by observations and interviews, to investigate what exists in a place like a public authority waiting room. Questions like what kind of interactions will take place and which dilemmas might occur are asked. The result shows that different approaches in how social workers do call clients up occur, the accessibility or lack of accessibility to computers, telephone and copying machine, clarifies the power imbalance that exists in governmental work. The result also shows that some waiting room interactions may be experienced as enforced and that the social services law is hard to maintain.

    The purpose of the study was to examine if intrinsic motivation and work commitment to the organization differs between permanent doctors and hired doctors. The total of 63 doctors who participated in the study made an internet-based survey with 21 questions taken from the Basic Need Satisfaction at Work Scale and 3 questions from the section “work commitment” in QPS Nordic. The main results of the study showed that permanent doctors perceived a higher level of intrinsic motivation on all the three needs autonomy, competence and relatedness. Furthermore the result showed a tendency that permanent doctors perceived a higher level of work commitment to the organization then hired doctors.

    Abstract [en]

    More and more people suffer from dementia and a large part of these is affected by some point of one or several behavioral and psychological symptoms of dementia (BPSD). BPSD creates great suffering for the affected but can also be difficult for the people around them to manage, relatives and health professionals, and is a big reason that a person moves in to a nursing facility. The aim of this study was to describe health professional’s experience of caring for persons with behavioral and psychological symptoms in dementia, BPSD. The study had a descriptive design with qualitative approach. Data were collected through six individual semi-structured interviews. By convenience sampling, nursing assistant working at a nursing facility for people with dementia in Sweden were recruited. Data were analyzed by qualitative content analysis. A theme emerged, “Difficulties to find a balance between need and resource”. The informants experienced their work in caring for people with BPSD as a great challenge but also as very awarding. The feeling of being inadequate, lacking knowledge or ability to handle certain situations or symptoms and lack of resources perceived as stressful by the informants. Aggressiveness and anxiety in individuals with BPSD were experienced as the most difficult to handle. The results also showed that the informants experienced working with persons with BPSD as exiting, interesting and fun and that they felt satisfaction in being there for the person with BPSD and to be able to contribute to their well-being. The conclusion in this study shows that informants experienced great challenges in their work to create wellbeing for people with BPSD. Support and education could increase the job satisfaction for the health personnel but also increase the quality of life for people with BPSD.

    Bakgrund: Antal fall av diabetes mellitus typ 2 ökar i Sverige samt över hela världen. Okunskap om hur sjukdomen utvecklas och hanteras kan leda till komplikationer och andra sjukdomar.

    Syfte: Syftet med litteraturstudien var att beskriva vuxna personers erfarenheter av att leva med diabetes mellitus typ 2, samt att beskriva de inkluderade artiklarnas datainsamlingsmetod.

    Metod: Litteraturstudien har deskriptiv design. Studien inkluderar 12 kvalitativa artiklar funna i databaserna PubMed och Cinahl.

    Huvudresultat: I analysen av artiklarna framkom kategorier som besvarade vuxna personers erfarenheter av att leva med diabetes mellitus typ 2. Kategorierna var: Erfarenheter av att ha diagnostiseras med diabetes typ 2, Vuxna personers erfarenheter av kontroll och behandling av diabetes typ 2, Erfarenheter av kostförändringar i samband med diabetes typ 2 hos vuxna personer, Erfarenheter av sociala sammanhang hos vuxna personer med diabetes typ 2 och Erfarenheter och hanteringssätt att leva med sin diabetes typ 2. Resultatet reflekterar positiva och negativa erfarenheter i det vardagliga livet i hos personer med diabetes mellitus typ 2.

    Slutsater: Att få diabetes mellitus typ 2 medförde många olika känslor hos flera informanter. Diabetes påverkade vardagliga livet och krävde förändringar i den drabbades liv. Flera informanter erfor frustation över vården och många erfor stöd från familj och vänner. Rädsla för att drabbas av komplikationer till följd av sin sjukdom och inte få se sina barn växa upp gav motivation till att ta hand om sin sjukdom.
