    Abstract [en]

    Flight baggage handlers are employed by handling companies engaged in sorting, loading and unloading luggage, cargo and mail, so called ramp service work. After check-in, luggage are transported through a sorting area and further out to the ramp. This transportation involves manual handling by baggage handlers at several stages, using conveyor belts, carts and trucks for transport. In addition to these tasks, baggage handlers are also engaged in communicating with air traffic controllers directing air traffic on the ground, towing aircrafts to gates and serving them with auxiliary power units, brakes and light. Baggage handling services are similar in all larger airports, and so baggage handlers perform similar tasks all over the world. In Sweden the handlers’ union claimed a high prevalence of low back and shoulder pain and a dissatisfying psychosocial work environment, but systematically collected empirical data were not available, and the literature was surprisingly sparse, considering that the occupation is global. Thus, a comprehensive project initiated by the Vocational Training and Working Environment Council, TYA, in Sweden - a council formed by employers’ and employees’ organizations in the transportation sector -was conducted at 14 handling companies in six Swedish airports between 2010 and 2012. The project aimed to document the physical and psychosocial work environment and to contribute to the development of ergonomic interventions within this occupation, which could, eventually, lead to better health among the employees. The present study was part of this project, in aiming at documenting psychosocial exposures and musculoskeletal disorders (MSDs) in the flight baggage handler population, and at determining possible associations between exposures and disorders.

    Physical and psychosocial work conditions among baggage handlers in six Swedish airports2012Konferensbidrag (Refereegranskat)
    Abstract [en]

    Introduction

    Flight baggage handlers are mainly engaged in sorting luggage or cargo, loading and unloading it to and from the airplanes. The Vocational Training and Working Environment Council, TYA - formed by employer’s and employee’s organizations in the transportation sector - initiated a scientific study in 2009 to investigate the prevalence of musculoskeletal disorders and their suspected determinants in six Swedish airports involving a total of about 1000 handlers in 14 cargo- and handling companies. Encouraged by an initial literature review, the present field study was designed to contain qualitative, questionnaire-based, and observational surveys of working conditions, as well as extensive direct measurements of postures using full-shift inclinometry. This paper reports the design and results of the questionnaire part of the study.

    Method

    All baggage handlers working at least half-time (n=1044) were encouraged to fill in an extensive questionnaire handed out at the workplace by a research team member. In general the researcher collected the questionnaires at the same occasion. The questionnaire addressed general health, work capacity and physical exposures in relevant handling tasks. It also included a modified version of the Copenhagen Psychosocial Questionnaire (COPSOQ), the Nordic Council of Minister’s Questionnaire (NMQ) on disorders, and the SOFI-questionnaire measuring perceived fatigue.

    Results

    The response rate was 73%. The prevalence of musculoskeletal disorders in the back, shoulders and wrists during the last 12 months was 70%, 60% and 45%. Positive effects of devices used for reducing perceived physical load were confirmed. The handlers expressed a low confidence in the leadership, and insufficient feedback, information and influence at work. Fatigue particularly occurred in the dimensions lack of energy and physical discomfort.

    Discussion

    The observed prevalence of low back pain (70%) is high, and in parity with results among nurses in Sweden (64%; Josephson et al. 1997) and China (56%; Smith et al. 2004). Further examination of questionnaires, interviews and direct posture measurements will identify determinants to consider for intervention to reduce the prevalence of disorders among the baggage handlers.

    Healthy work - healthy business? A survey of work environment improvements and profitability among small and medium enterprises2010Konferensbidrag (Refereegranskat)
    Abstract [en]

    The aim was to investigate how Swedish small and medium enterprises (SMEs) within the manufacturing industry carry out work environment improvements, and to study if managers consider profitability to be associated with issues in the work environment. Workplace observations and interviews (n=60) with company managers were carried out. Forty-five enterprises were affiliated with an occupational health service, typically in order to get quick access to health care, rather than getting help planning and implementing changes. Fifty-eight managers believed that healthy work environments would have at least some positive effect on company profitability. Incentives for work environment improvements were external, individual and organizational and varied between companies of different sizes. External requirements from The Swedish Work Environment Authority and from established or potential customers were prioritized incentives in SMEs with < 50 employees. Organizational and individual incentives such as incident- and accident reports, productivity, health and sick-leave were prioritized in SMEs with >50 employees. Irrespective of company size, well-being among work-force was consistently mentioned as an important factor for successful business. Small companies urged quick decisions and actions while medium companies had more planned, systematic and organized work environment activities. Deliberate work environment improvements for increased profitability were for instance, changes to eliminate risks, reduce manual work and resolve planning and coordination problems in production that led to stress.

    Bakgrund

    Psykisk ohälsa är en vanlig orsak till lidande och försämrad arbetsförmåga både i Sverige och internationellt. Effektiva insatser för att förebygga eller minska psykisk ohälsa är därför av stor betydelse för både folkhälsa och arbetshälsa. Företagshälsovården (FHV) är en viktig aktör på svenska arbetsplatser då majoriteten av alla anställda har tillgång till denna tjänst. Kunskapen är dock begränsad vad gäller effekter av de förebyggande eller rehabiliterande insatser som ges av FHV kring psykisk ohälsa. Denna litteratur-genomgång publicerades först 2015 och har nu uppdaterats med studier publicerade fram t.o.m. maj 2017.

    Syfte

    Att genomföra en systematisk kartläggning av nationell och internationell forskning där insatserna getts av företagshälsovården för att förebygga eller minska psykisk ohälsa i arbetslivet.

    Metod

    Litteratursökningar gjordes i fyra databaser och det vetenskapliga underlaget kom att utgöras av 33 studier varav 12 är nytillkomna i denna uppdatering. Prospektiva studier med eller utan jämförelsegrupp inkluderades. Urval av studier och kvalitetsgranskning gjordes av två oberoende forskare.

    Resultat

    I 18 av studierna utvärderades rehabiliterande insatser som riktade sig till an-ställda sjuk-skrivna p.g.a. psykisk ohälsa, i elva studier insatser till anställda i risk för psykisk ohälsa och i 4 studier utvärderades förebyggande insatser. Drygt hälften av de inkluderade stud-ierna (17 studier) var från Nederländerna. Tjugoen av 33 studier bedömdes ha medelhög eller hög kvalitet. För rehabiliterande insatser som ges till anställda sjukskrivna för psykisk ohälsa indikerar det vetenskapliga underlaget att problemlösningsbaserad metod och KBT med arbetsplatsinriktning minskar sjukskrivning och/eller påskyndar arbetsåtergång jämfört med sedvanlig insats. Effekten är oklar för övriga rehabiliterande insatser. Kunskapsläget är oklart gällande effekter av förebyggande insatser och effekter av insatser som ges till anställda i risk för psykisk ohälsa. Några av studierna tyder på positiva effekter men insatserna som utvärderats är olika och det är viktigt att försök till replikering görs av dessa utvärderingar. Det finns ett behov av att förebyggande insatser utvärderas med ett forskningsupplägg som ökar möjligheterna att dra slutsatser om effekter. Flera studier indikerar också att det inte finns en tydlig relation mellan omfattning av symtom och arbetsåtergång. Detta understryker vikten av att vid insatser aktualisera arbetsåtergång tidigt i processen och att erbjuda metoder för samtidig symtomhantering och arbetsåtergång.Sammantaget indikerar resultaten att arbetsplatsinriktade insatser med KBT eller problemlösningsbaserad metod som ges via FHV kan förkorta tid till arbetsåtergång bland anställda sjukskrivna för psykisk ohälsa och att särskilt förebyggande insatser behöver utvecklas och utvärderas ytterligare.

    Abstract [en]

    Background: Common mental disorders affect about one-third of the European working-age population and are one of the leading causes of sick leave in Sweden and other OECD countries. Besides the individual suffering, the costs for society are high. This paper describes the design of a study to evaluate a work-related, problem-solving intervention provided at primary health care centers for employees on sick leave due to common mental disorders. Methods: The study has a two-armed cluster randomized design in which the participating rehabilitation coordinators are randomized into delivering the intervention or providing care-as-usual. Employees on sick leave due to common mental disorders will be recruited by an independent research assistant. The intervention aims to improve the employee's return-to-work process by identifying problems perceived as hindering return-to-work and finding solutions. The rehabilitation coordinator facilitates a participatory approach, in which the employee and the employer together identify obstacles and solutions in relation to the work situation. The primary outcome is total number of sick leave days during the 18-month follow-up after inclusion. A long-term follow-up at 36 months is planned. Secondary outcomes are short-term sick leave (min. 2 weeks and max. 12 weeks), psychological symptoms, work ability, presenteeism and health related quality of life assessed at baseline, 6 and 12-month follow-up. Intervention fidelity, reach, dose delivered and dose received will be examined in a process evaluation. An economic evaluation will put health-related quality of life and sick leave in relation to costs from the perspectives of society and health care services. A parallel ethical evaluation will focus on the interventions consequences for patient autonomy, privacy, equality, fairness and professional ethos and integrity. Discussion: The study is a pragmatic trial which will include analyses of the intervention's effectiveness, and a process evaluation in primary health care settings. Methodological strengths and challenges are discussed, such as the risk of selection bias, contamination and detection bias. If the intervention shows promising results for return-to-work, the prospects are good for implementing the intervention in routine primary health care.     Discourses about electricians in vocational education2018Konferensbidrag (Refereegranskat)
    Abstract [en]

    Students in vocational education to become electricians have an increased risk of developing musculoskeletal disorders (MSDs) once in the workforce compared to most other blue collar professions (Toomingas et al., 2014). This increased risk comes from having to work in awkward body positions, with heavy and sometimes poorly adapted tools and with time constraints. Many end up with chronic disabilities, forcing them to change careers or to go on sickness pension. Besides the significant and damaging consequences for the individual concerned, losing electricians from the workforce is a significant loss for the building industry in which electricians these days are scarce, and for society that not only loses tax-incomes but also possibly need to pay sickness pension for these people during quite a few years. It is therefore important to find ways to prevent MSDs and to promote good ergonomics in the building industry in general and among electricians specifically. Electricians’ knowledge and habits regarding MSDs are first formed and shaped in school, during vocational education to become an electrician. Drawing on the ideas of bio-power and governmentality, as introduced and developed by Foucault (1988, 1990, 2003) and developed by Rose (1999; Rabinow and Rose, 2006), discourses govern how it is possible to think and act. From this standpoint, then, talk and discourses are not perceived as innocent or ‘mere’ talk (Hall, 2001) but as governing the production, regulation and representation of both bodies and subjects through the acquisition of specific dispositions, tastes and abilities (Foucault, 1988; Rose, 1999). Thus, discourses drawn on in school about electricians and the profession govern how it is possible to think and act about oneself and others in relation to both ergonomics and MSD and, by extension, have material effects on electricians’ health. This means that, to foster healthy ergonomics one needs to consider how electricians and the profession are conceived and thought about in school, i.e. how electricians and the profession are discursively conceived. The purpose of this paper is thus to explore discursive constructions of electricians and the profession as these are expressed in discussions about MSDs in the vocational education and school context. Methods used are four focus group interviews: two with students, one with teachers and one with school management at one senior high school program for electricians. In the focus group interviews the participants were asked to discuss ideas about causes and reasons to why electricians develop MSDs, consequences and effects of MSDs and ideas about what could be done to prevent MSDs. The interviews were then transcribed and discursively analyzed with questions in mind about how each group conceived of electricians and the profession.

Besides negative consequences for the individuals concerned, MSDs also have a detrimental impact on the workforce (reduced number of professionals) and on society (financial costs). It is therefore important to find ways to prevent MSDs and to promote good ergonomic behavior. Electricians’ knowledge and habits regarding MSDs are first formed and shaped in school, during vocational education to become an electrician. In the course of this time the students have periods of internship during which they interact with professionals within the field. Drawing on the ideas of bio-power and governmentality, as introduced and developed by Foucault and developed further by Rose, discourses govern how it is possible to think and act. From this standpoint, then, talk and discourses are not perceived as innocent or ‘mere’ talk but as producing and regulating subjects through their acquisition of specific dispositions, tastes and abilities. Thus, discourses drawn on about electricians and the profession during the students’ internship will govern how it is possible for the students to think and act about themselves and others in relation to both ergonomic behavior and MSDs and, by extension, will have material effects on electricians’ health. This means that, to foster healthy ergonomic behavior one needs to consider how electricians are thought of within the field, i.e. how electricians and the profession are discursively conceived. The purpose of this paper is thus to explore discursive constructions of electricians and the profession as these were expressed in discussions about MSDs among professionals in the field.  Two semi-structured focus group interviews and one individual interview with professionals in the field were conducted. The participants were asked to discuss ideas about i) causes as to why electricians develop MSDs, ii) consequences and effects of MSDs, and iii) ideas about what could be done to prevent MSDs. The interviews were transcribed and the material is currently being discursively analyzed with questions in mind about how the participants conceived of electricians and the profession. Results will be presented at the conference, but preliminary analyses suggests that predominant discourses are concerned with the masculinity of electricians as well as with their time pressured work situation, both contributing to casualness towards ergonomic behavior.

    Abstract [en]

    Context: Stretching is common within sports as a potential maneuver for injury prevention. Stretching induced changes in muscle spindIe properties is a suggested mechanism. This may imply a reduction in proprioception following stretching, however, little is known of this association. Our finding showing no effect of acute stretching on shoulder position sense provides insight into this issue.

    Objectives: To evaluate if acute stretching of the shoulder muscles affects position sense.

    Design: A crossover design with subjects randomized to 3 groups, as regarded by the I sequence of 3 interventions.

    Setting: A university human research laboratory.

    Patients or Other participants: Nine female (age, 21 +2) and 9 male (24 + 3) healthy volunteers.

    lntervention(s): The interventions consisted of stretching of shoulder 1) agonists, 2) antagonists, and 3) non-stretching control.

    Main Outcome Measure(s): Position sense acuity of the right shoulder was determined before and arter the interventions by subjects at tempting to reproduce arm positions of 15° and 30° (shoulder adduction) while starting at 45° to the sagittal plane. The outcome variables were the response variability (variable error, VE) and overall accuracy (absolute error, AE).

    Results: A multivariate repeated measures analysis ofvariance revealed that the relative change in VE (i.e., VE after/VE before) was not significantly different between the interventions (p = 0.38). Similarly no change in AE was found (p = 0.76). Furthermore, there were no differences regarding test sequence or in the interaction 'intervention x sequence' for either VE (p = 0.73 and 0.53, respectively) or for AE (p = 0.71 and 0.67, respectively)

    Conclusions: The present study showed no effect on shoulder position sense after an acute bout of stretching either agonist or antagonist shoulder muscles.

    Effects of tailored versus non-tailored treatment on pain and pressure pain threshold in women with nonspecific neck pain: a randomized controlled trial2014Konferensbidrag (Refereegranskat)
    Abstract [en]

    Aim of the investigation: The evidence for physiotherapy treatments of nonspecific neck pain is modest despite a large increase of intervention studies the last decade. One reason could be different underlying causes for pain in individuals with nonspecific neck pain, and that identification of sub-groups or individual needs is seldom accounted for in studies. In the absence of causal treatment options, a tailored treatment approach based on an explicit clinical decision model guided by assessment of function, clinical signs and symptoms, should be considered. Our aim was to evaluate tailored treatment based on such a decision model, targeting women with nonspecific neck pain. Our main hypothesis was that the tailored treatment (T) would have better short, intermediate and long-term effects on pain intensity and pressure pain threshold for the trapezius muscles than either non-tailored treatment (NT) (same treatment components but applied quasi-randomly) or treatment-as-usual (TAU) (no treatment from the study, no restrictions). We further hypothesized that T or NT has better effect than TAU. For details, cf. Current Controlled Trials registration ISRCTN49348025 and published study protocol.

    Methods: 120 working women with minimum six weeks duration of neck pain were randomized to the T, NT or TAU groups. All participants had more than “no disability” but less than “complete disability” according to the Neck Disability Index, and reported impaired capacity on the quality or quantity to work the preceding month. Main exclusion criteria were trauma-related neck pain, specific diagnoses and generalized pain or concomitant low back pain. The decision model for tailored treatment was based on tests and symptoms with defined cut-off levels comprising the following main categories: reduced cervical mobility, impaired neck-shoulder strength and motor control, trapezius myalgia, cervicogenic headache and impaired eye-head-neck control (cf. published study protocol). Assessment was performed one week before and after the 11-weeks intervention, with follow-ups 6-months (intermediate-term) and 12-months (long-term) after the intervention. Outcome variables were pain intensity (Numeric Rating Scale, NRS, 0 – 10) and pressure pain threshold (PPT) of the upper trapezius muscles (kPa). PPT was not measured at long-term follow-up.  Preliminary statistical analyses for the predefined hypotheses were performed with analysis of covariance (ANCOVA) with baseline outcome values as covariates. This was supplemented with pairwise Bonferroni-compensated comparisons in case of significance of factor group.

    Results: 86% of the participants completed the intervention, and the attrition was similar across groups. Preliminary results for the short term evaluation showed a reduction in NRS from an average of 4.4 and 4.5 to 2.5 in the T and NT groups, respectively, which was significantly greater compared to the TAU group (p=0.024 and p=0.014 for T and NT). For the PPT, there was no difference between T and NT groups at the short term evaluation, but close to a significantly increased threshold for the T compared to the TAU group (p=0,058). No differences were found between treatment groups on the intermediate and long-term evaluations for neither of the two outcome variables.

    Conclusions: The results indicate that tailored treatment for women with nonspecific neck pain may not be more effective, with respect to pain reduction, compared to non-tailored treatment. The hypothesis of superiority of tailored or non-tailored treatment over treatment-as-usual was partly supported for the short-term evaluation. However, the short-term results should be interpreted with caution since the impact of higher attention given to the participants in T and NT groups is not known. Reference:1.     Many of the existing low back pain (LBP) questionnaires of function and symptoms have a content of different domains of disability presented as a single sum score, making it difficult to derive changes within a specific domain. The present study describes the development of a clinically derived back-specific questionnaire incorporating both a functional limitation and a symptom scale, with a further subdivision of the symptom scale in separate indices for severity and temporal aspects. The aims of the study were to assess the overall reliability and validity of the new questionnaire, named the Profile Fitness Mapping questionnaire (PFM). A total of 193 chronic LBP patients answered the PFM together with five validated criterion questionnaires. For the internal consistency of the questionnaires, the three indices of the PFM had the highest Cronbach's alpha (0.90-0.95) and all items had item-total correlations above 0.2. The correlation coefficients between the PFM and the back-specific criterion questionnaires ranged between 0.61 and 0.83, indicating good concurrent criterion validity. The best discriminative ability between patients with different pain severities was demonstrated by the functional limitation scale of the PFM. Well centered score distribution with no patient's score at the floor or the ceiling level indicates that the PFM has the potential to detect the improvement or worsening of symptoms and functional limitations in chronic LBP patients. Classification according to the International Classification of Functioning, Disability and health (ICF) of WHO revealed a high degree of homogeneous item content of the symptom scale to the domain of impairments, and of the functional limitation scale to the domain of activity limitations. The present study suggests that the PFM has a high internal consistency and is a valid indicator of symptoms and functional limitations of LBP patients. It offers the combination of a composite total score and the possibility of evaluations within specific domains of disability. Complementary evaluation of test-retest reliability and responsiveness to change is warranted.

    Background: Disability questionnaires for LBP-patients mostlyemanate from health professionals and have a content of differentdomains of disability presented as a single sum score, makingit difficult to derive changes within a specific domain.     Abstract [en]

    BACKGROUND: Disability questionnaires for LBP-patients mostly emanate from health professionals and have a content of different domains of disability presented as a single sum score, making it difficult to derive changes within a specific domain. This study introduces a new back-specific questionnaire, the Profile Fitness Mapping questionnaire (PFM), which was based on patient’s self-reported characteristics of the LBP. The PFM incorporates both a functional limitation and a symptom scale, with further subdivision of the symptom scale in separate indices for severity and temporal aspects. The aim of the study was to assess the overall validity of the PFM.

    METHODS AND RESULTS: Chronic LBP-patients (n=193) answered the PFM and four validated back-specific criterion questionnaires. The correlation coefficients between the PFM and the criterion questionnaires showed good concurrent criterion validity (0.61 – 0.83). The best discriminative ability between patients with different pain severity was demonstrated by the functional limitation scale of the PFM. Classification according to the ICF revealed a high degree of homogeneous item content of the symptom scale to the domain of impairments, and of the functional limitation scale to the domain of activity limitations. Well centered score distribution indicates that the PFM has the potential to detect improvement or worsening of symptoms and functional limitations in chronic LBP-patients.

    CONCLUSION: The results of the study signify that the PFM is a valid indicator of symptoms and functional limitations of LBP-patients. It provides the combination of a composite total score and the possibility of evaluations within specific domains of disability.

    Abstract [en]

    Purpose: To assess overall reliability and validity of a neck-specific questionnaire, the Profile Fitness Mapping neck questionnaire (ProFitMap-neck), on three chronic neck pain groups. Method: Participating groups were as follows: whiplash associated disorders, inpatient care (IP-WAD, n = 127); nonspecific neck pain, inpatient care (IP-NS, n = 83) and nonspecific neck pain subjects (non-IP-NS, n  = 104). All groups answered the ProFitMap-neck and the SF-36, whereas non-IP-NS also answered the Neck Disability Index (NDI) and the Functional Self-Efficacy Scale (SES). Internal consistency, test–retest reliability and components of convergent construct, face and content validity were determined for the ProFitMap-neck. Results: The ProFitMap-neck showed good internal consistency in all three groups, and ICC test–retest reliability (0.80–0.91). Good correlation (0.66–0.78) and highest agreement was reached with NDI. According to the International Classification of Functioning, Disability and Health, the symptom scale of the ProFitMap-neck was mainly classified to the domain of impairments–body functions, and the functional limitation scale to the activity limitation domain. Conclusion: The results indicate that the ProFitMap-neck is valid for measuring symptoms and functional limitations in people with chronic neck pain. The combination of a composite total score of symptoms and function as well as separate scores of each domain makes ProFitMap-neck suitable for research as well as in clinical practice.

    Implications for Rehabilitation

    • The ProFitMap-neck can be used as a valid self-assessment tool for measuring symptoms and functional limitations in people belonging to the most prevalent categories of neck pain.

    • The combination of the symptom and functional limitation questionnaire scores in a total score can be used for an overall clinical judgment.

    Effects of combined mechanical and nociceptive stimuli on nociceptive-responsive dorsal horn neurons of cat spinal cord2000Konferensbidrag (Refereegranskat)
    Abstract [en]

    The effects of large ramp and hold stretch of the gastrocnemius muscles were studied on dorsal horn nociceptive-responsive neurons (DHN) in decerebrated cats. The stretch consisted of 3 subsequent plateaus, ending with a release phase from a near maximal physiological length of the muscle. This was done in order to mimic the "therapeutical stretches" used in clinical settings to alleviate muscle tenderness and stiffness. Stretch was applied before and after i.a. injection of bradykinin (50 microg.) into the gastrocnemius muscles. Changes in DHN activity were extracellularly recorded with high impedance glass microelectrodes. Only neurons up to approximately 1 mm were recorded. Neurons were identified as nociceptive on the basis of their response to an identification procedure applied on beforehand. Responsiveness of the DHN to stretch was variable. If responsive, a typical behavior was an increased firing rate during the ramp phases, followed by a gradual decrease during the hold phases and a reduction in firing rate or a complete silence during and after the release phase, usually lasting up to 50 s. Bradykinin injections induced either excitation or inhibition of DHN.. Stretch applied directly after the bradykinin injection in most cases elicited changes in the bradykinin-induced response of the DHN, changing both response profile as well as its strength. Results indicate that nociceptive-responding DHN could be influenced by innocuous ramp and hold stretch both in conjunction with and without chemically induced activation. The behavior of the DHN to stretch in the presence of the painful stimuli may be one of the possible mechanisms that could explain the short term relieving effects of muscle stretching when applied on stiff and tender muscles.

These functions may be associated with emergence and retention of neck pain in cases where the neck strength does not meet daily demands. However, clear empiric support for these notions are lacking, partly due to a scarcity of well controlled dynamometry studies with large samples. First, clarification is needed whether neck strength is in fact reduced in people with neck pain and to resolve the diagnostic performance of neck strength tests.Purpose: To compare neck muscle strength of women with non-specific long-term neck pain and healthy controls. Also, to assess the diagnostic performance of neck strength tests by assessing their discriminative ability to discern women with and without neck pain.Methods: The study had a cross-sectional design with data on cervical strength derived from the baseline measurement of a randomized controlled trial (RCT). 80 women with long-term non-specific neck pain (NP) were compared with 40 healthy women (CON). The NP group was a subsample from the RCT selected so that there were no group differences (NP-CON) for body weight and physical activity. Cervical strength assessment included dynamometry of cervico-thoracic extension (CTE) and flexion (CTF) in sitting and cranio-cervical flexion (CCF) in standing, all performed with isometric maximum voluntary contraction (iMVC). Independent samples Mann-Whitney U test and T-test were used to assess group differences for iMVC of the tests. Diagnostic accuracy was further assessed with a receiver operating characteristic (ROC) curve by plotting the true positive rate (sensitivity) as a function of the false positive rate (1 - specificity). The area under the ROC curve (AUC) with 95% confidence interval was used to determine discriminative ability of the tests. The optimal cut-off value to discern NP from CON with corresponding sensitivity and specificity was also determined.Results: Women with neck pain produced significant lower iMVC in CTE (28%), CTF (26%) and CCF (33%) (all p< 0.001). The ability of CTE, CTF and CCF to discriminate between NP and CON showed moderate accuracy (AUC 0.83, 0.78 and 0.73, respectively). The cut-off value of 165.7 N in CTE had a sensitivity of 0.725 and a specificity of 0.8. The corresponding values for CTF and CCF were 85.8 N (sensitivity 0.8; specificity 0.692) and 4.2 Nm (sensitivity 0.575; specificity 0.9).Conclusion(s): The neck pain group had less neck muscle strength than controls in all tests. The diagnostic performance of the neck strength tests, judged as discriminative ability to discern neck pain from control participants, were moderate. These results support earlier findings of reduced neck strength in people with neck pain. Also, neck strength tests may have a complementary value in the assessment of neck pain persons.Implications: The results highlights that impaired neck strength, both in global cervico-thoracic and deep cranio-cervical muscles, is most likely a characteristic of people with long-term neck pain.     Office-cycling while working: An innovative concept to prevent and reduce musculoskeletal pain in office workers - a controlled feasibility study2015Konferensbidrag (Refereegranskat)
    Abstract [en]

    Background: According to the World Health Organization, WHO, a sedentary lifestyle is the single largest health risk for a number of diseases including musculoskeletal disorders and metabolic diseases. The negative health effects of excessive sitting are not compensated for by shorter bouts of increased physical activity. However, evidence shows that increased physical activity reduces musculoskeletal pain, which is very prevalent in those who are inactive. About 50-70 % of those who work at a computer report musculoskeletal pain and spend on average about 5 hours/day with very low energy metabolism. Work places are therefore an important arena for prevention and intervention by means of reducing sedentary time and increasing physical activity both for general health benefits and effects on the musculoskeletal pain.

    Purpose: To test the feasibility of office-cycling in an office work place and explore its potential effects on musculoskeletal pain in office workers.

    Methods: Twenty office workers (ages 27-61, 5 males) with musculoskeletal pain participated in this three-week controlled pilot field study. The intervention group (n=10), had access to an innovative customized cycle ergometer (OfficeBiking®) at their regular office workstation whilst performing their usual work tasks. Offie-cycling was an alternative to sitting/standing by their height adjustable office desk; they were instructed to bike as often as comfortable. The control group (n=10) was instructed to continue to work as usual. The experiences of office-cycling and how it influenced work performance was studied with a questionnaire. Musculoskeletal pain was evaluated using pain drawings and pain ratings and participants' total pain was calculated by adding each individuals' self-reported pain from their three most painful areas (NRS 0-10).

    Results: Importantly, office-cycling did not reduce self-reported work performance; the majority (9/10) would like daily access; and made suggestions to improve the user-friendliness of the bike. Office-cycling was used regularly (median, 11/15 workdays; median active time 59 min/day IQR 39;91). There was no observed difference regarding either number of self-reported areas of pain (NSAP) or general musculoskeletal pain (GMP) between the intervention group and the control group at baseline. Self-reported GMP decreased in 8 persons in the intervention group which was one more than in the control group (n=7). NSAP decreased in the intervention group (n=7; md -1,0 IQR -2,3;0,0); and the control group (n=5; md -0,5 IQR -1,3;0,3). The difference in total pain (intervention end-baseline) revealed a clinically important change in the intervention group (NRS -2,5, IQR -8,8;4,0) but not in the control group (NRS 0,0 IQR -6,2;2,5).

    Conclusions: The results suggest that office-cycling is a feasible method for use in work place interventions with some promising results. Future research suggestions are: underlying mechanisms regarding effects of physical activity on pain in parallel with controlled studies in laboratory environments to investigate dose-effects for metabolic expenditure and optimal pain reduction whilst office-cycling.

    Implications: The results in this feasibility study indicate a promising potential of the innovative office-cycling concept to prevent and reduce musculoskeletal pain in sedentary office workers.

    Purpose

    The aim was to determine the responsiveness and minimal important change (MIC) of the questionnaire ProFitMap-neck that measures symptoms and functional limitations in people with neck pain. The same measurement properties were determined for Neck Disability Index (NDI) for comparison purposes.

    Methods

    Longitudinal data were derived from two randomized controlled trials, including 103 and 120 women with non-specific neck pain, with questionnaire measurements performed before and after interventions. Sensitivity and specificity to discriminate between improved and non-improved participants, based on categorization of a global rating of change scale (GRCS), were determined for the ProFitMap-neck indices and NDI by using area under receiver operator curves (AUC). Correlations between the GRCS anchor and change scores of the questionnaires were also used to assess responsiveness. The change score that showed the highest combination of sensitivity and specificity was set for MIC.

    Results

    The ProFitMap-neck indices showed similar responsiveness as NDI with AUC exceeding 0.70 (Range: ProFitMap-neck, 0.74 – 0.83; NDI, 0.75 – 0.86). The MIC in the two samples ranged between 6.6 – 13.6% for ProFitMap-neck indices and 5.2 and 6.3% for NDI. Both questionnaires had significant correlations with GRCS (Spearman’s rho 0.47 – 0.72).

    Conclusions

    Validity of change scores was demonstrated for the ProFitMap-neck indices with adequate ability to discriminate between improved and non-improved participants. Values of minimal important change were presented.

    BACKGROUND: The shortage of nurses in operating room departments (ORs) in Sweden and other countries can lead to reduced capacity and quality in healthcare, as well as more intense work for those on the job. Little is known about what nurses in ORs perceive as crucial for their workplace to be attractive.

    OBJECTIVE: To capture attractive qualities of nurses work in ORs and to adapt the Attractive Work Questionnaire (AWQ).

    METHODS: The AWQ, rating attractive qualities of work, were completed by 147 (68%) nurses in four Swedish ORs. Principal Component Analyses were performed to determine the underlying structure of the data.

    RESULTS: The factors in the area Work conditions were: relations, leadership, equipment, salary, organisation, physical work environment, location, and working hours; in the area Work content: mental work, autonomy and work rate; and in the area Job satisfaction: status and acknowledgement. The Principal Component Analysis showed consistency with the original AWQ. Cronbach’s alpha varied between 0.57-0.90.

    CONCLUSIONS: The AWQ captured attractive qualities for nurses in ORs, some less discussed regarding nurse retention, i.e. equipment, physical work environment and location. The results suggest that the questionnaire is reliable and valid and can be a useful tool in identifying attractive work.

    AIMS AND OBJECTIVES: The aim was to examine significant factors for work attractiveness and how these differ from the current work situation among operating department nurses. A second objective was to examine the associations between age, gender, length of employment, work engagement, work ability, self-rated health indicators and attractiveness of the current work situation.

    BACKGROUND: The attractiveness of work is rarely taken into account in research on nurse retention. To expand this knowledge, it is relevant to examine factors that make work attractive and their associations with related concepts.

    DESIGN: Correlational, cross-sectional survey using a convenience sample.

    METHODS: Questionnaires were answered by 147 nurses in four operating departments in Sweden. Correlation and regression analyses were conducted.

    RESULTS: The nurses rated the significance of all factors of work attractiveness higher than they rated those factors in their current work situation; salary, organisation and physical work environment had the largest differences. The most significant attractive factors were relationships, leadership and status. A statistically significant positive correlation between work engagement and attractive work was found. In the multiple regression model, the independent variables work engagement and older age significantly predicted work attractiveness.

    CONCLUSIONS: Several factors should be considered in the effort to increase work attractiveness in operating departments and thereby to encourage nurse retention. Positive aspects of work seem to unite work engagement and attractive work, while work ability and self-rated health indicators are other important dimensions in nurse retention.

    RELEVANCE TO CLINICAL PRACTICE: The great discrepancies between the significance of attractive factors and the current work situation in salary, organisation and physical work environment suggest ways in which work attractiveness may be increased. To discover exactly what needs to be improved may require a deeper look into the construct of the examined factors.

    Both effectiveness and good care is demanded of nurses in a complex environment. The aim of the study was to describe how nurse managers and nurses interpret and are guided by stated goals in their health-care organisation, their own influence over work and obstacles to carry out work in an operating department. A qualitative study, consisting of interviews with nurse managers and nurses, and a description of goal documents and the workplace, were conducted in an operating department in Sweden. The interviews were analysed with manifest content analysis. Work was guided by daily goals for the nurses: to meet the operating schedule and to ensure good-quality patient care. The organisational goals were little known and used. The nurse managers and nurses had limited influence over their work, changes in assignments and in the operating schedule, and unsuitable environmental premises created obstacles in their work. If organisational goals are to guide work there is a need for nurse managers to transform them into understandable, applicable goals and incorporate them into nurses’ daily work. Letting the nurses influence the operating schedule could be one way to overcome obstacles in work. The study also highlights the importance of a functional physical work environment.

    Abstract [en]

    Background To improve working conditions and counteract nursing shortages, more knowledge is needed about the opportunities nurses have to carry out their work and what guide nurses’ work. The aim of this study was to use the ARIA guide to identify the opportunities and obstacles in nurse managers, registered specialist nurses and non-registered assistant nurses work in an operating department. Methods A descriptive case study design was performed in an operating department, comprising structured individual and group interviews according to work content analyses with nurse managers, registered specialist nurses and assistant nurses. Interviews were analysed in terms of predetermined aspects regarding working conditions and criteria for performance obstacles. Extracts from documents stating goals and registered data supplemented the interviews. Results The findings show that the daily surgery schedule, not the goal documents, guided daily work. An over-optimistic surgery schedule with unplanned changes and cancellations, over which the nurses had very little influence, as well as the time required to locate necessary equipment that was spread throughout the ward, resulted in nurses rushing through medical records and other preparations rather than preparing calmly for surgery. Although the registered specialist nurses and assistant nurses considered quality of care to be highly important, no standardised evaluations on quality of care were performed. Conclusion The study reveals the importance of a functioning physical work environment including storage, technical equipment supplies, and the positioning of technical equipment in operating rooms, in order for registered specialist and assistant nurses to perform their tasks well. Due to goal incongruence and performance obstacles, the nurses were often unable to reach their daily goals. Involving registered specialist nurses in the process of planning of the surgery schedule could facilitate their work to better match the prerequisites in the physical work environment and among available staff. In order for the Operating Department Goal Document to guide work, goals must be transformed into understandable, realistic, applicable and evaluable aims, and incorporated into daily work.

    Abstract [en]

    Methods of statistical physics have been recently successfully ap- plied to the study of spatial ánd temporal randomness in various biological systems, like the analýsts of the DNA nucleotide sequences and the heartbeat time series. We study the interspike intervals (ISI) time series of the spinal dorsal horn nociceptive- responsive neurons (DHN) activity in decerebrate cats applying the detrended fluctuatìon analysis (DFA) that is a modification of the standard random walk model analysis. Specifically, we focus on ISI variability as an important quantity to help elucidate sensory coding and signal processing performed by DHN. DFA has been applied for it permtts quantification of correlation properties of a nonstationary time series of neuronal discharge, Changes in DHN activity were extracellularly recorded with high impedance glass microelectrodes from superficial laminae of the dorsal horn, during different experimentally simúlated conditions. We háve analyzed DHN discharge patterns during spontaneous activity, as well as in the presence of diflerent noxious and non- noxious mechanical stimuli. Application of DFA method showed significant changes in dynamics of neural discharge when the ex- ternal stimulus is appliéd. These findings demonstrate the relevance of the application of methods of statistical. physics to identify the changes in aflerent inflow, and to characterize temporal patterns of activity of neurons under study

    Objectives To develop predictive models for short-term and long-term clinically important improvement in women with non-specific chronic disabling neck pain during the clinical course of physiotherapy. Design Longitudinal cohort study based on data from a randomised controlled trial evaluating short-term and long-term effects on sensorimotor function over 11 weeks of physiotherapy. Participants and settings Eighty-nine women aged 31-65 years with non-specific chronic disabling neck pain from Gavle, Sweden. Measures The outcome, clinically important improvement, was measured with the Patient Global Impression of Change Scale (PGICS) and the Neck Disability Index (NDI), assessed by self-administered questionnaires at 3, 9 and 15 months from the start of the interventions (baseline). Twelve baseline prognostic factors were considered in the analyses. The predictive models were built using random-effects logistic regression. The predictive ability of the models was measured by the area under the receiver operating characteristic curve (AUC). Internal validity was assessed with cross-validation using the bootstrap resampling technique. Results Factors included in the final PGICS model were neck disability and age, and in the NDI model, neck disability, depression and catastrophising. In both models, the odds for short-term and long-term improvement increased with higher baseline neck disability, while the odds decreased with increasing age (PGICS model), and with increasing level of depression (NDI model). In the NDI model, higher baseline levels of catastrophising indicated increased odds for short-term improvement and decreased odds for long-term improvement. Both models showed acceptable predictive validity with an AUC of 0.64 (95% CI 0.55 to 0.73) and 0.67 (95% CI 0.59 to 0.75), respectively. Conclusion Age, neck disability and psychological factors seem to be important predictors of improvement, and may inform clinical decisions about physiotherapy in women with chronic neck pain. Before using the developed predictive models in clinical practice, however, they should be validated in other populations and tested in clinical settings.

    Abstract [en]

    The expression of alpha(1a)-adrenoreceptors (alpha(1a)-ARs) within the muscle spindles of rabbit masseter muscle was investigated. The alpha(1a)-ARs were detected by immunohistochemical fluorescent method and examined along the entire length of 109 cross serially sectioned spindles. The sympathetic fibers were visualized by the immunofluorescent labeling of the noradrenaline synthesizing enzymes tyrosine hydroxylase (TH) and dopamine beta-hydroxylase (DBH). In order to recognize the intrafusal muscle fiber types, antibodies for different myosin heavy chain isoforms (MyHCI) were used. TH and DBH immunolabeled nerve fibers have been observed within the capsule lamellar layers, in the periaxial fluid space and close to intrafusal muscle fibers. The alpha(1a)-ARs were detected on the smooth muscle cells of the blood vessels coursing in the muscle and in the capsule lamellar layers or within the periaxial fluid space of the spindles. Moreover, at the polar regions of a high percentage (88.1%) of muscle spindles a strong alpha(1a)-ARs immunoreactivity was present on the intrafusal muscle fibers. In double immunostained sections for alpha(1a)-ARs and MyHCI it was evidenced that both bag, and nuclear chain fibers express alpha(1a)-ARs. The receptors that we have detected by immunofluorescence may support a direct control by adrenergic fibers on muscle spindle.

    Background. Perceived muscular tension (PMT) has been suggested to predict development of neck/shoulder pain. It has been hypothesized to be an early sign of musculoskeletal disorder and a possible mediator of stress on symptoms. However, the content of the concept of PMT is not clear. This study examined the association between PMT and physical and psychosocial factors and physical activity in a group of healthy students.

    Methods. This cross-sectional study was conducted on the baseline measurements of an ongoing longitudinal case-control study. A total of 63 healthy university students without complaints of neck/shoulder pain were included (21 males, 42 females, mean age 24 years). PMT was measured by asking the question “Have you, during the past month, experienced muscular tension (for example, wrinkled your forehead, ground your teeth, raised your shoulders)?” with the following response options: never, a few times, a few times per week, or one or several times per day. Self-reports on symptoms in the neck, anxiety, depression, stress, mental health, physical health, sleep and physical activity were collected with questionnaires, as well as by tenderness on palpation of neck muscles and trapezius pressure pain threshold. This produced a total of 15 variables. The relationship between these variables and PMT were analyzed using Spearman’s rank correlation coefficient.

    Results. Positive correlations were found between PMT and temporomandibular complaints (rho= .34, p < .001), neck crepitus (rho= .33, p < .001), anxiety (rho= .33, p < .001), depression (rho= .31, p < .05), tenderness on palpation (rho= .25, p < .05). There was a negative correlation between PMT and mental health (rho= -.26, p < .05). Frequent experience of PMT had weak to moderate correlations with frequency of symptoms and higher psychosocial strain, but not with stress. This suggests some covariance between PMT and both physical and psychosocial factors.

    This study investigated temporal changes in movement strategy and performance during fatiguing short-cycle work. Eighteen participants performed six 7-minutes work blocks with repetitive reaching movements at 0.5 Hz, each followed by a 5.5-minute rest break for a total duration of one hour. Electromyography (EMG) was collected continuously from the upper trapezius muscle, the temporal movement strategy and timing errors were obtained on a cycle-to-cycle basis, and perceived fatigue was rated before and after each work block. Clear signs of fatigue according to subjective ratings and EMG manifestations developed within each work block, as well as during the entire hour. For most participants, timing errors gradually increased, as did the waiting time at the near target. Changes in temporal movement strategy were negatively correlated with changes in the level and variability of EMG, suggesting that an adaptive temporal strategy offset the development of unstable motor solutions in this fatiguing, short-cycle work

    This study investigated the effect of work pace on workload, motor variability and fatigue during light assembly work. Upper extremity kinematics and electromyography (EMG) were obtained on a cycle-to-cycle basis for eight participants during two conditions, corresponding to "normal" and "high" work pace according to a predetermined time system for engineering. Indicators of fatigue, pain sensitivity and performance were recorded before, during and after the task. The level and variability of muscle activity did not differ according to work pace, and manifestations of muscle fatigue or changed pain sensitivity were not observed. In the high work pace, however, participants moved more efficiently, they showed more variability in wrist speed and acceleration, but they also made more errors. These results suggest that an increased work pace, within the range addressed here, will not have any substantial adverse effects on acute motor performance and fatigue in light, cyclic assembly work. STATEMENT OF RELEVANCE: In the manufacturing industry, work pace is a key issue in production system design and hence of interest to ergonomists as well as engineers. In this laboratory study, increasing the work pace did not show adverse effects in terms of biomechanical exposures and muscle fatigue, but it did lead to more errors. For the industrial engineer, this observation suggests that an increase in work pace might diminish production quality, even without any noticeable fatigue being experienced by the operators.

    Physical variation is generally considered to be an important factor influencing the risk for musculoskeletal disorders in repetitive work, but a comprehensive scientific basis for this assumption has not been available. Thus, the Swedish Work Environment Authority requested the Centre for Musculoskeletal Research at the University of Gävle to review scientific standings regarding physical variation and its effects.  In total, 56 articles were included in the review. The results showed that occupationally relevant studies of the effects of physical variation are few, and that the effectiveness of initiatives promoting variation has also been studied to a limited extent. Thus, current research cannot provide a clear answer to what an effective combination would be of work tasks in a job in the context of physical variation, let alone the optimal time distribution of tasks in a short (hours, days) and long (weeks, months, years) perspective. Also, gender aspects of physical variation were considered to a very limited extent. There is a need for more studies of relevant initiatives aiming at creating increased physical variation by changing the contents of work or its temporal structure; including studies placing this issue in a gender perspective.

    Many schoolchildren frequently use computers. Lack of postural variation is proposed as a risk factor for musculoskeletal disorders (MSDs) in adult computer users, but the effect of computer use on children's postural t variation is unknown. This study exarnined if there was a reduction in postural variability among schoolchildren when ! using computers compared to other tasks. Nine schoolchildren were observed in their natural environment while upper body postures were measured using inclinometers. Tasks performed and type of technology used was documented by an observer and matched to postural data with a rninute-to-rninute resolution. A comparison was made of postures during New Information and Communication Technology (ICT) tasks ( electronic-based), Old ICT (paperbased), and Non ICT tasks. Mean postures were deterrnined and postural variation was characterised using an index based on the Exposure Variation Analysis (EV A) matrix, and the range between the 10th and 90th percentiles of the Amplitude Probability Distribution Function (APDF). New ICT produced more neutral postures but significantly lower postural variation. Old ICT had less neutral postures but greater postural variation. The relationship between lack of variation and MSDs among children requires further investigation.

    Background: Musculoskeletal disorders have been associated with computer use among office workers and lack of variation caused by too little exposure diversity between work tasks has been proposed as an important etiological factor. However there is little information on the diversity of occupations and information and communication technologies (ICT) used by office workers and none which extends beyond the traditional workday. Whilst direct observation is expected to provide the most accurate data, it is resource intensive and self report and sampling alternatives may be a viable alternative.

    Method: This paper describes direct observation and self-report data on the occupations and ICT use of 24 Australian office workers in their natural environments at work and away-from-work, over 12 hours of a working day.

    Results: Participants were observed for a mean [SD] for 642[40] minutes, 67% of which was at the workplace. Productive occupations (405[122] minutes) accounted for 63% of the observation period, compared to 17% for instrumental (106[57] minutes), 12% for self-care (75[46] minutes) and 8% for leisure occupations (54[39] minutes). Non ICT tasks occurred during 44% of the observation period (285[89] minutes); New ICT accounted for 36 % (234[118] minutes), Old ICT accounted for 15% (98[73] minutes), and Combined ICT tasks 4% (24[30] minutes). The proportions of occupations and ICT use differed between work and away-from-work. Computer-based New ICT was most used for work tasks. Observed and self-reported time on occupations and ICT were similar, though with some differences. Self-reported time on occupations and ICT was similar for the first day and the subsequent 4 days.

    Conclusion: The first detailed description of occupations and ICT used by office workers at work and away-from-work shows that the variety of Old, New, Combined and Non-ICT used for performing typical work and non-work tasks offered a diversity of exposures for the workers. This information provides an important step for further investigations into exposure variation in this group, and possible musculoskeletal health risks.

    Abstract [en]

    Background: Reports in the popular media are that school children use modern information and communication technology (ICT) on a regular basis for a variety of purposes, however little has been documented in the scientific literature about how school children spend their time and the different types of ICT they use.

    Method: This paper describes the observed occupations and ICT use of nine Australian primary school children in their natural environments at school and away-from-school during one school day, and compares self-reported exposures with direct observations. Self-reported discomfort scores were obtained throughout the day.

    Results: The study identified that paper-based ICT (Old ICT) was used mostly for productive occupations at school, while electronics-based (New ICT) was used mostly during leisure in away-from-school locations. Tasks involving no ICT (Non ICT) accounted for the largest proportion of time in both locations during self-care, leisure and instrumental occupations. End-of-day self-reported time performing different occupations was consistent with data from independent observations. Self reported time using Old ICT and New ICT was marginally over-estimated, and time spent using Non-ICT was marginally under-estimated.

    Conclusion: The children in this study used a variety of ICT in the performance of daily occupations in their natural environments. New ICT use was primarily for leisure, but time spent was less than reported among other child studies. Discomfort reports among the participants were low. Children’s self-reports of daily occupations and ICT use has utility as an exposure assessment metric.

    Background: There are concerns that insufficient variation in postural and muscle activity associated with modern information and communication technology (ICT) tasks presents a risk for musculoskeletal ill-health among school children. However, scientific knowledge on physical exposure variation in this group is limited.

    Method: Postures of the head, upper back and upper arm, and muscle activity of the right and left upper trapezius and right forearm extensors were measured over 10-12 hours in nine school children using different types of ICT at school and away-from-school. Variation in postures and muscle activity was quantified using two indices, EVAsd and APDF(90-10).

    Results: Paper-based (Old) ICT tasks produced postures that were less neutral but more variable than electronics-based (New ICT) and Non-ICT tasks. Non-ICT tasks involved mean postures similar to New ICT tasks, but with greater variation. Variation of muscle activity was similar between ICT types in the right and left upper trapezius muscles. Non-ICT tasks produced more muscle activity variation in the right forearm extensor group compared to New and Old ICT tasks.

    Conclusion: Different ICT tasks produce different degrees of variation of postures and muscle activity. Combining tasks that use different ICT may increase overall exposure variation. More research is needed to determine what degree of postural and muscle activity variation is associated with reduced risk of musculoskeletal ill-health.

    Office workers perform tasks using different information and communication technologies (ICT) involving various postures. Adequate variation in postures and muscle activity is generally believed to protect against musculoskeletal complaints, but insufficient information exists regarding the effect on postural variation of using different ICT. Thus, this study among office workers aimed to determine and compare postures and postural variation associated with using distinct types of ICT. Upper arm, head and trunk postures of 24 office workers were measured with the Physiometer® over a whole day in their natural work and away-from-work environments. Postural variation was quantified using two indices; APDF(90-10) and EVA(sd).Various ICT had different postural means and variation. Paper-based tasks had more non-neutral, yet also more variable postures. Electronics-based tasks had more neutral postures, with less postural variability. Tasks simultaneously using paper- and electronics-based ICT had least neutral and least variable postures. Tasks without ICT usually had the most posture variability. Interspersing tasks involving different ICT could increase overall exposure variation among office workers and may thus contribute to musculoskeletal risk reduction.

    Objective: To determine differences in muscle activity amplitudes and variation of amplitudes, when using different Information and Communication Technologies (ICT).

    Background: Office workers use different ICT to perform tasks. Upper body musculoskeletal complaints are frequently reported by this occupational group. Increased muscle activity and insufficient muscle activity variation are potential risk factors for musculoskeletal complaints.

    Method: Muscle activity of right and left upper trapezius and right wrist extensor muscle bundle (extensor carpi radialis longus and brevis) of 24 office workers (performing their usual tasks requiring different ICT at work and away-from-work) were measured continuously over 10-12 hours. Muscle activity variation was quantified using two indices, APDF(90-10) and EVAsd.

    Results: There was a trend for electronics-based New ICT tasks to involve less electromyography (EMG) variation than paper-based Old ICT tasks. Performing Combined ICT tasks (i.e. using paper- and electronics-based ICT simultaneously) resulted in the highest muscle activity levels and least variation; however, these Combined ICT tasks were rarely performed. Tasks involving no ICT (Non-ICT) had the greatest muscle activity variation.

    Conclusion: Office workers in this study used various ICT during tasks at work and away-from-work. The high EMG amplitudes and low variation observed when using Combined ICT may present the greatest risk for musculoskeletal complaints, and use of Combined ICT by workers should be kept low in office work. Breaking up Combined, New and Old ICT tasks; for example, by interspersing highly variable Non-ICT tasks into office workers’ daily tasks, could increase overall muscle activity variation and reduce risk for musculoskeletal complaints.

    Aim: Occupational physical activity (OPA), and particularly static postures and physically exerting activities, is known to impact worker health and to increase the risk of cardiovascular disease, musculoskeletal problems, sickness absence and premature retirement. The exploration of structural preventive measures at the workplace against the adverse health effects of excessive OPA is needed. The psychosocial work environment is hypothesised to buffer the adverse effects of OPA, and as such psychosocial resources might directly influence the performance of OPA. However, this has not been previously investigated with detailed objective measurements. The aim of this study is to describe OPA within blue-collar workers, and to examine the role of psychosocial job resources in the performance of OPA.

    Methods: Results are based on a sample of 198 blue-collar workers from the NOMAD (New method for Objective Measurements of physical Activity in Daily living) study, recruited from seven workplaces in Denmark. The sample included 112 men (56.6%) and 86 women (43.4%); the mean age was 44.9 years (SD 9.9). Data were collected with two Actigraph devices placed on the thigh and trunk, during four consecutive days. The accelerometer data were processed and analysed using the Acti4 software, to determine working time spent standing, walking, on feet and in activity of moderate to vigorous intensity level (MVPA). The level of influence and social support at work were assessed by questionnaire, and measured with a four-item scale. Analysis of (co-)variance and (multiple) linear regression models were conducted. All analyses were stratified by gender predominance of occupation.

    Results: The different types of OPA significantly varied by particular job type. Within male predominant occupations, job type accounted for 50–70% of explained variance, depending on the type of OPA. Manufacturing workers showed the highest average proportions of working time standing (33%) and on feet (79%), while garbage collectors had the highest proportion of working time in MVPA (33%). Mobile plant operators and construction workers had the lowest average working time spent walking and in MVPA. Differences in OPA between job types in female predominant occupations were less pronounced, but healthcare workers and cleaners had higher average proportions of time spent walking and in MVPA compared to assembly workers. The addition of age and psychosocial resources to the models did not contribute to a larger explained variance in OPA and the relations with job type remained significant. Social support at work showed an independent positive relation with working on feet, and with standing in female predominant jobs only. Influence at work was not related to OPA.

    Conclusion: The positive relation of social support with working on feet and standing is likely to be explained by the nature of the work tasks, as jobs that require these activities probably comprise more close interactions and as such create more intensified levels of cooperation at the work floor. Overall, our hypothesis that psychosocial job resources would affect the performance of OPA within blue-collar workers was not confirmed. These findings suggest that the performance of OPA within blue-collarjobs – and particularly within male predominant occupations – is mostly affected by work organisational factors related to specific job type, and not by psychosocial job resources.

    The aim was to describe occupational physical activity (OPA) and examine the role of psychosocial job resources among blue-collar workers. In a sample of 198 workers (57% male; mean age 44.9 (SD 9.9) year) from 7 companies in Denmark, two accelerometers (Actigraph) were placed on the thigh and trunk during 1-5 consecutive days, to determine working time spent standing, walking, on feet and in activity of moderate to vigorous intensity level (MVPA). The level of influence and social support at work were assessed by questionnaire. The exposure to OPA significantly varied by particular job type, especially in male predominant occupations. Overall, psychosocial job resources did not affect the exposure to OPA. These findings suggest that workplace interventions aiming to prevent adverse outcomes of OPA among blue-collars workers ought to focus on task redesign and target work organizational factors related to specific job type.

    Recent studies suggest that while leisure-time physical activity (LTPA) promotes general health, engaging in occupational physical activity (OPA) may have negative health consequences. It has been hypothesized that the different health effects from OPA and LTPA can be explained by differences in physical activity (PA) intensity in these two domains. To assess the intensity of OPA and LTPA, we aimed to study the percentage heart rate reserve (%HRR) during similar types of OPA and LTPA during workdays. Data from the NOMAD study on Danish blue-collar workers (n=124) with objective measurements of PA (using accelerometers) and heart rate (using heart rate monitors) for 4 workdays were analysed. Activities of sitting, standing, moving, walking, and stair climbing were identified and %HRR in each of these activities was determined for work and leisure. %HRR was significantly higher during OPA than LTPA. These differences were more pronounced in men than in women. Although not statistically significant in the fully adjusted model, we found indications that these differences were more pronounced in those with low compared to high fitness. To our knowledge, this is the first study with objective measurements showing that %HRR is higher during the same gross-body postural activities when performed at work compared to leisure-time during workdays. This elevated intensity may help explaining the negative health consequences of engagement in high levels of OPA. Future guidelines should distinguish OPA from LTPA, possibly by advising workers to remain active during their leisure time, in particular when they are highly active at work.

    Aim: Physical activity (PA) is considered to be an important factor in the prevention of various cardiovascular diseases. However, recent studies suggest that while leisure time PA promotes cardiovascular health, occupational PA might impair cardiovascular health. An explanation for this PA health paradox may be a difference in the intensity and associated physical demands between occupational and leisure time PA. Occupational PA often consists of low-intensity, long-lasting physically demanding tasks, such as repetitive work and prolonged static working postures, which are presumed to cause sustained elevated heart rate that may stress the cardiovascular system. Despite this notion, the differences in physiological responses between occupational and leisure time PA are not well understood. Therefore, we aimed: (a) to study the difference in intensity of occupational and leisure time PA (expressed in percentage heart rate reserve; % HRR); and b) to assess whether this potential difference varies by gender and cardiorespiratory fitness level.

    Methods: We used data from the NOMAD study, in which Danish blue-collar workers from seven different workplaces took part in a four-day protocol of objective measurements of PA (using hip and thigh-worn accelerometers) and heart rate (using an ambulatory heart rate monitor). During occupational and leisure time, activities of sitting, standing, moving, walking and stair climbing were identified, and %HRR in each of these activities was determined. Differences in %HRR between occupational and leisure time PA were tested using generalised estimating equations (expressed in regression coefficient – beta with 95% confidence interval (CI)) adjusted for personal, health,work and lifestyle confounders.

    Result: In 124 workers with data on PA and heart rate, %HRR was higher for occupational PA compared to leisure time PA (beta1.9, 95% CI2.4,1.4,P<0.001). Differences in %HRR between occupational and leisure time PA were more pronounced in men than in women, and in those with high cardiorespiratory fitness compared to those with low cardiorespiratory fitness.

    Conclusion: This study is the first to assess differences in %HRR between occupational and leisure time PA, using objectiv emeasurements in blue-collarworkers. Cardiovascular intensity was higher in occupational activities (possibly due to additional physical and/or mental workloads) compared to the same activities during leisure time. The increase in cardiovascular intensity at work maybe a contributing factor to the health paradox of occupational and leisure time PA, suggesting negative cardiovascular health consequences for engagement in occupational PA (see Figure 1).
