Bakgrund: Psykiatrisk tvångsvård ges till personer som lider av allvarliga psykiska störningar och inte frivilligt samtycker till behandling, men där vården bedöms som nödvändig. Tvångsåtgärder som kan användas under tvångsvård inkluderar fasthållning, bältesläggning, avskiljning, tvångsmedicinering och kvarhållning på vårdenheten. Tvångsvård kräver en balansgång mellan patientens rätt till autonomi och behovet av vård. Sjuksköterskans uppgift blir att ge omvårdnad och stödja patienten, och samtidigt utöva auktoritet och kontroll för patientens bästa, samt hantera de etiska dilemman som uppstår vid tvångsvård och tvångsåtgärder.

Syfte: Att beskriva patienters upplevelser av tvångsvård och tvångsåtgärder inom psykiatrisk slutenvården.

Metod: En beskrivande litteraturstudie genomfördes med en innehållsanalys av 14 kvalitativa artiklar.

Huvudresultat: Patienterna upplevde känslor av maktlöshet och kontrollförlust samt annat psykiskt lidande, ofta förvärrat av bristande kommunikation och stöd från vårdpersonal. Samtidigt ansåg vissa patienter att tvångsvården var nödvändig för att hjälpa och skydda dem eller andra. Vårdpersonalens attityd och bemötande var avgörande för patienternas upplevelser, där empati och respekt kunde minska de negativa effekterna av tvång.

Slutsats: För att förbättra patienternas upplevelser av tvångsvård är det viktigt att stärka kommunikationen och ge vård som respekterar patientens autonomi och värdighet.

Background: Involuntary psychiatric care is provided to people suffering from severe mental disorders who do not voluntarily consent to treatment, but where care is deemed necessary. Coercive measures that can be used during involuntary care include restraint, seclusion, forced medication, and confinement to the care unit. Involuntary care requires a balance between the patient's right to autonomy and the need for treatment. The nurse's role is to provide care and support to the patient, while simultaneously exercising authority and control in the patient's best interest and managing the ethical dilemmas that arise in involuntary care and the use of coercive measures.

Aim: To describe patients' experiences of involuntary care and coercive measures in psychiatric inpatient care.

Method: A descriptive literature review was conducted with a content analysis of 14 qualitative articles.

Main Findings: Patients reported feelings of powerlessness and loss of control, along with other forms of psychological distress, often exacerbated by poor communication and lack of support from healthcare staff. At the same time, some patients acknowledged that involuntary care was necessary to help and protect themselves or others. The attitude and approach of healthcare staff were crucial to patients' experiences, with empathy and respect helping to reduce the negative effects of coercive measures.

Conclusion: To improve patients' experiences of involuntary care, it is essential to enhance communication and provide care that respects the patient's autonomy and dignity.
