Sammanfattning
Bakgrund: Psykiatrisk tvångsvård används när personer med allvarliga psykiatriska tillstånd bedöms ha ett oundvikligt behov av vård men motsätter sig denna. Vården regleras av lagstiftning och syftar till att skydda patientens liv och hälsa, men innebär samtidigt en begränsning av patientens autonomi, integritet och värdighet. Detta kan väcka etiska frågor och påverka hur vården upplevs av patienterna.
Syfte: Syftet med studien var att belysa patienters upplevelser av psykiatrisk tvångsvård.
Metod: En beskrivande litteraturstudie baserad på 10 kvalitativa vetenskapliga artiklar identifierade genom systematiska databassökningar i PubMed och CIHNAL. Artiklarna genomgick en relevans- och kvalitetsgranskning och analyserades enligt en tematisk analysprocess.
Huvudresultat: Tre huvudteman med tillhörande subteman identifierades. Tvångsvård präglas av känslor av maktlöshet och förlorad kontroll, centrala aspekter i dessa upplevelser är bristande autonomi, maktskillnader samt förekomst av informellt tvång. Vidare framkommer att bemötande, relationer och tillgång till information har stor betydelse för patienters vårdupplevelse. Tvångsvården kan även leda till omfattande negativa emotionella och psykosociala konsekvenser. Samtidigt kunde en ambivalens framträda i patienternas upplevelser, där vissa i efterhand beskrev tvångsvården som nödvändig eller betydelsefull.
Slutsats: Denna studie visar att psykiatrisk tvångsvård kan förstås som en balans mellan nödvändigt vårdbehov och respekt för patientens autonomi, där upplevelsen påverkas av bemötande och information. Sjuksköterskan har en central roll i att bidra till en mer respektfull vård och främja patientens hälsa och välbefinnande.

Abstract
Background: Psychiatric involuntary treatment is used when individuals with severe psychiatric conditions are considered to have an unavoidable need for care but refuse treatment. Although such care aims to protect the patient’s health and safety, it may restrict autonomy, integrity and dignity, which can influence how patients experience the care.
Aim: The aim of this study was to describe patients´ experience of psychiatric involuntary treatment.
Method: A descriptive literature review based on 10 qualitative scientific articles identified through systematic database searches in PubMed and CIHNAL. The articles underwent a relevance and quality review and were analyzed according to a thematic analysis process.
Main results: Three main themes with associated subthemes were identified. Involuntary treatment is characterized by feelings of powerlessness and loss of control, central aspects of these experiences are loss of autonomy, power imbalances and the occurrence of informal coercion. Furthermore, it appears that treatment, relationships and access to information are of great importance for patients' experiences of care. Involuntary treatment can also lead to extensive negative emotional and psychosocial consequences. At the same time, ambivalence could appear in the patients' experiences, with some retrospectively describing the involuntary treatment as necessary or significant.
Conclusion: This study shows that psychiatric involuntary treatment can be understood as a balance between necessary care needs and respect for the patients´ autonomy, where the experience is influenced by treatment and information. The nurse has a central role in contributing to more respectful care and promoting the patient's health and well-being.
