Substans abuse among older adults – A challenge in practical social work? (English)

Alcohol consumption among older adults is increasing, yet related problems often remain hidden because symptoms are mistaken for signs of normal aging. This study investigates the barriers and challenges social workers face when supporting older adults (65+) with alcohol problems, as well as the interventions currently available and the need for their further development. The study is guided by two research questions: what obstacles social workers encounter, what interventions are used, and whether these require adaptation. The theoretical framework combines critical theory, emphasizing structural conditions and power relations; socialization theory, explaining how norms and identity shape alcohol use; theories on aging and alcohol, highlighting biological and social vulnerabilities; and the concept of a health-promoting work environment, which situates social workers’ working conditions as vital to sustainable practice. Using a qualitative method, data were gathered through nine semi-structured interviews with social workers and care managers in a Swedish municipality. Analysis followed grounded theory principles, identifying recurring themes through coding and categorization. Findings indicate that alcohol problems among older adults are often detected late. Stigma and self-stigma limit help-seeking, while organizational divisions between eldercare and addiction services hinder effective support. Existing interventions are typically designed for younger clients and fail to meet older adults’ needs. Social workers report ethical stress, unclear responsibilities, and insufficient guidelines. The study concludes that sustainable and effective support requires addressing both individual and structural factors, developing age-appropriate interventions, improving cross-sector collaboration, and strengthening social workers’ work environments.
